慢性肺心病患者急性发作期血清酶与血氧分压的关系

The correlation of serum enzyme and blood oxygen pressure in acute outbreak period of chronic cor pulmonale patient

目的 :研究慢性肺心病患者急性发作期血清谷丙转氨酶 (ALT)、谷草转氨酶 (AST)、乳酸脱氢酶 (LDH)、( -羟基丁酸脱氢酶 (HBDH)、磷酸肌酸激酶 (CPK)与动脉血氧分压的关系。方法 :采用酶催化L -丙氨酸与α -酮戊二酸测定ALT ;采用酶催化天冬氨酸与α -酮戊二酸测定AST ;采用酶催化α -丁酸与NADH的反应测定HB DH ;采用酶催化磷酸肌酸与ADP的反应法测定CPK ;采用Bayer550血气分析仪测定动脉血氧分压。采用SPSS10.0进行数据相关分析。结果 :急性期肺心病患者动脉血氧分压显著低于缓解期肺心病患者 (P=0.0001)。急性期肺心病患者血清ALT、AST、LDH、HBDH显著高于缓解期患者 (P=0.026 ,P=0.040 ,P=0.003 ,P=0.0001) ,而CPK差异无显著性 (P=0.107)。急性发作期肺心病患者血清ALT、AST、LDH、HBDH均与动脉血氧分压呈现显著负相关关系 (r= -0.425、P=0.005;r= -0.481、P=0.001 ;r= -0.468、P=0.002 ;r= -0.479、P=0.001)。结论 :肺心病的急性发血氧分压与血清ALT、AST、LDH、HBDH有负相关关系。

Objective:To study the correlation of serum enzyme and blood oxygen pressure in acute outbreak period of chronic cor pulmonale patient.Methods:The method of enzyme catˉalyze the reaction of L-alanine andα-ketoglutaric acid was used to determined the level of ALT in serum,at the same time,the method of enzyme catalyze the reaction of aspartic acid andα-ketoglutaric acid was used to determined the level of AST in serum.Serum CPK was determined by the method of catabolic reaction of phosphocreatine and ADP.Bayer550air-blood instrument was used to determine the oxygen pressure of blood.Results:Oxygen pressure of the acute outbreak group was significantly lower that that of the alleviate group(P=0.0001).The ALT,AST,LDH and HBDH levels of serum in the acute outbreak period of cor pulmonale group were higher than that of the alleviate group(P=0.026,P=0.040,P=0.003,P=0.0001),but there was no difference of serum CPK between the two groups(P=0.107).The correlations of oxygenpressure of artery and serum ALT,AST,LDH and HBDH were notable negative(r=-0.425,P=0.005,r=-0.481,P=0.001,r=-0.468,P=0.002,r=-0.479,P=0.001).Conclusion:There were negative correlations of oxygen pressure and serum enzymes in patients of chronic cor pulmonale who were in acute outbreak period,and the low oxygen pressure maybe the cause of the increase.
