POCT联合检测cTnI和NT-proBNP在急性心力衰竭中的临床应用

The Clinical Application of Combined Detection of cTnI and NT-proBNP Used by POCT in Patients with Acute Heart Failure

目的:总结POCT联合检测cTnI和NT-proBNP急性心力衰竭(AHF)中的临床价值。方法:以广东省东莞市大朗医院2016年10月-2018年10月间诊治的非急性心肌梗死患者AHF47例为研究组,以同期诊治的非急性心肌梗死的非AHF患者30例为对照组,比较c TnI和NT-proBNP在两组中的变化及其在研究组中的改变。结果:研究组c TnI和均明显高于对照组[(0.031±0.016)μg/L vs(0.019±0.012)μg/L,P<0.05]和[(3856.42±1915.51)ng/L vs(794.58±362.27)ng/L,P<0.05)];cTnI、NT-proBNP、cTnI/NT-proBNP、c TnI+NT-proBNP诊断AHF率分别是63.8%、89.4%、95.7%、85.1%;cTnI单独诊断AHF的敏感性与特异性分别为64.01%、95.23.0%;NT-proBNP单独诊断AHF的敏感性与特异性分别为88.96%、91.24%;cTnI/NT-proBNP诊断AHF的敏感性与特异性分别为94.91%、85.93%,cTnI+NT-proBNP诊断AHF的敏感性与特异性分别83.88%、99.13%。结论:急诊联合POCT联合检测cTnI和NT-proBNP有助于排除AHF,避免AHF的漏诊。

Objective:To sumarize the clinic value of combined detection of cTnI and NT-proBNP in patients with acute heart failure(AHF).Methods:47 patients with AHF caused by no acute myocardiac infarction(no-AMI)diagnosed and treated in Dalang Hospital of Guangdong Province from Oct 2016 to Oct 2018 were classified as the research group,while the 30 patients with cardiovascular diseases combined with no-AMI were classified as control group.The changes of cTnI and NT-proBNP were compared in the two groups and the changes of its in the research group.Results:The levels of c TnI and NT-proBNP in the research group were significantly higher than those in the control group[(0.031±0.016)μg/L vs(0.019±0.012)μg/L,P<0.05]and((3856.42±1915.51)ng/L vs(794.58±362.27)ng/L,P<0.05)The diagnostic rate of AHF of cTnI,NT-proBNP,c TnI/NT-proBNP and cTnI+NT-proBNP was 63.8%,89.4%,95.7%and 85.1%respectively.The sensitivity and specificity of diagnose of AHF of cTnI alone was 64.01%and95.23.0%,while the NT-proBNP was 88.96%and 91.24%.The sensitivity and specificity of diagnose of AHF of c TnI/NTproBNP was 94.91%and 85.93%,while the cTnI+NT-proBNP was 83.88%and 99.13%.Conclusion:It is help of exclude to AHF for the combined detection of cTnI and NT-proBNP and avoid the missed diagnosis of AHF.
