脑梗通汤辨证治疗气虚血瘀型脑梗死恢复期偏瘫的临床疗效

Clinical Effect of Naogengtong Decoction for Treating Qi Deficiency and Blood Stasis Type of Cerebral Infarction Hemiplegia

目的:研究脑梗通汤辨证治疗气虚血瘀型脑梗死恢复期偏瘫的临床效果。方法:选择2015年7月—2017年3月在本院进行康复治疗的100例脑梗死恢复期气虚血瘀型偏瘫患者,采用随机数字表法随机均分为两组。对照组采用舒血宁注射液治疗;观察组在此基础上配合脑梗通汤进行治疗。观察记录两组治疗效果,采用简式Fugl-Meyer评定量表和日常生活活动能力Barthel指数评定表评价患侧肢体运动功能和日常生活能力,并检测比较两组患者治疗前后血液流变学指标变化情况。结果:观察组患者临床总有效率98.00%明显高于对照组80.00%,差异有统计学意义(P<0.05)。治疗后,两组患者FMA评分、ADL评分明显上升;且观察组FMA和ADL评分明显高于对照组(P<0.05)。两组患者治疗后,血液流变学指标均明显改善;观察组全血黏度、血浆纤维蛋白原、红细胞聚集指数及血浆黏度均明显低于对照组,血液流变学改善程度明显优于对照组(P<0.05)。结论:脑梗通汤辨证治疗气虚血瘀型脑梗死恢复期偏瘫患者,能有效改善其偏瘫肢体功能和日常生活能力,改善患者血液流变学,提高患者生存质量。

Objective:To study the clinical effect of Naogengtong Decoction for treating Qi deficiency and blood stasis type of cerebral infarction hemiplegia.Methods:From July 2015 to March 2017,100 patients with hemiplegia at the convalescent stage of cerebral infarction in our hospital were randomly divided into two groups.The control group was treated by Shuxuening Injection.On the basis, the observation group was treated with Naogengtong Decoction.We observed and recorded the treatment effect of the two groups by the simple Fugl-Meyer scale and Barthel ADL index evaluation of limb motor function and ability of daily living and detected two groups′ blood rheology indexes before and after treatment.Results:The total effective rate of the observation group was 98%,which was higher than that of the control group(80%).The difference was statistically significant(P<0.05).After treatment,the FMA and ADL scores of the two groups increased significantly and the scores of FMA and ADL in the observation group were significantly higher than those in the control group(P<0.05).Two groups′ blood rheology indexes were significantly improved after treatment.The observation group′s blood viscosity,plasma fibrinogen,erythrocyte aggregation index and plasma viscosity were significantly lower than those of the control group.The blood rheology improved significantly better than that of the control group(P<0.05).Conclusion:Naogengtong Decoction combined with Shuxuening Injection in the treatment of cerebral infarction of Qi deficiency and blood stasis in patients with hemiplegia can effectively improve the hemiplegic limb function and daily life ability,blood rheology and the quality of life of patients.
