基于关联规则和熵聚类算法的林毅治疗Ⅳ期乳腺癌用药规律研究

Analysis of Professor LIN Yi's Herbal Administration Experience for Stage Ⅳ Breast Cancer through Association Rules and Entropy Clustering

目的:基于中医传承辅助系统(V2. 5)探讨林毅对Ⅳ期乳腺癌中医用药经验及组方规律。方法:收集与整理2009年3月-2017年12月林毅在广东省中医院接诊的Ⅳ期乳腺癌医案,经过筛选后的医案共280条,将这些医案通过中医传承辅助系统(V 2. 5),使用关联规则分析和熵聚类等现代技术,最终得到林毅治疗Ⅳ期乳腺癌的辨证和处方用药规律。结果:共录入280个处方,145味中药,通过对这280个处方进行分析,统计出林毅治疗Ⅳ期乳腺癌的常用中药出现的频次,以及药物之间的关联规则,其中使用频次高于90的中药共14味,支持度为135,置信度为0. 96时,共有药物组合30组,此外还挖掘出治疗本病的12个核心配伍和6个新处方。结论:根据数据分析从临床实际证实了林毅正气亏虚是乳腺癌复发转移的先决条件的理论。林毅在治疗中确立扶正为主、祛邪为辅的治疗原则,始终把扶正固本放在首位。重视对中焦脾胃的调治,无论标本缓急,均重视脾胃的治疗原则。

Objective:To explore the herbal administration experience of Professor LIN Yi treating stageⅣbreast cancer by using the TCM inheritance system(V 2.5)software.Methods:We collected and screened the prescriptions which professor LIN used for treating stageⅣbreast cancer from March 2009 to December 2017 in Guangdong Hospital of TCM.By using Traditional Chinese Medicine Inheritance System,we established a formula database.Based on association rules,we revised mutual information,complex system entropy cluster,the principle experience and rules were analyzed and summarized.Results:We enrolled 280 cases and 145 herbs.Through analysis,the frequency of each herb and association rules of the herbs included in the database were identified.A total of 14 herbs had a frequency more than 90.There were 30 combinations of related herbs with degree of supporting level of 135 and the confidence level of 0.96.In addition,we also found 12 core compatibilities of medicines and 6 new prescriptions.Conclusion:Professor LIN considers the deficiency of positive Qi is the prerequisite of breast cancer recurrence and metastasis.We must establish the conception that strengthening the body in the primary position and think highly of Qi of the spleen and stomach in middle energizer.
