加味平肝降压汤联合硝苯地平治疗老年冠心病合并高血压的疗效及对血压、心功能的影响

Effect of Jiawei Pinggan Jiangya Decoction Combined with Nifedipine on Elderly Coronary Heart Disease Complicated with Hypertension and Its Effect on Blood Pressure and Heart Function

目的:研究加味平肝降压汤联合硝苯地平治疗老年冠心病合并高血压的疗效及对血压、心功能的影响。方法:选择2017年1月—2017年12月期间在本院接受诊治的108例冠心病合并高血压老年患者作为观察对象,参照随机数表法分成对照组、研究组,各54例。对照组患者采用硝苯地平治疗,研究组患者采用加味平肝降压汤联合硝苯地平治疗。对比两组患者临床疗效及治疗前后血压水平、心功能变化。结果:研究组患者的治疗总有效率94. 4%(51/54)高于对照组的75. 9%(41/54),差异有统计学意义(P <0. 05);两组患者治疗前的血压指标(DBP、SBP)水平差异无统计学意义(P> 0. 05);治疗后,两组患者的DBP、SBP水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P <0. 05);治疗后,两组患者的LVEF水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P <0. 05);两组患者的LVESD、BNP、LVEDD水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P <0. 05);两组患者术后不良事件发生率对比[7. 4%(4/54) vs13. 0%(7/54)],差异无统计学意义(P> 0. 05)。结论:加味平肝降压汤联合硝苯地平治疗老年冠心病合并高血压的疗效显著,能有效缓解临床症状,控制血压水平稳定,改善患者心功能,值得临床推广。

Objective:To study the effect of Jiawei Pinggan Jiangya Decoction combined with nifedipine on elderly patients with coronary heart disease complicated with hypertension and its influence on blood pressure and heart function.Methods:A total of 108 patients with coronary heart disease complicated with hypertension in our hospital from January 2017 to December 2017 were selected as the subjects of this study.All the patients were divided into a control group and a study group according to the random number table method,with 54 cases in each group.The patients in the control group were treated with nifedipine,and the patients in the study group were treated with Jiawei Pinggan Jiangya Decoction combined with nifedipine.The clinical efficacy,changes in blood pressure and heart function before and after treatment were compared between the two groups.Results:The total effective rate of treatment in the study group was 94.4%(51/54)higher than that in the control group(75.9%,41/54),and the difference was statistically significant(P<0.05).There was no significant difference in the blood pressure indexes(DBP,SBP)between the two groups before treatment(P>0.05).After treatment,the DBP and SBP levels in both groups were lower than those before treatment,and the study group’s was lower than that of the control group and the difference was statistically significant(P<0.05).The LVEF levels in the two groups were higher than that before treatment,and the study group’s was higher than that of the control group,and the difference was statistically significant(P<0.05).The levels of LVESD,BNP and LVEDD in the two groups were lower than that before treatment,and the study group’s was lower than that of the control group(P<0.05).There was no significant difference in the incidence of adverse events between the two groups[7.4%(4/54)vs13.0%(7/54)]and the difference was statistically significant(P<0.05).Conclusion:Jiawei Pinggan Jiangya Decoction combined with nifedipine has significant curative effect on elderly patients with coronary heart disease and hypertension.It can effectively relieve clinical symptoms,control blood pressure level and improve the heart function of patients.It is worthy of clinical promotion.
