复方麝香注射液联合高压氧对中等量高血压基底节区脑出血患者脑水肿及神经功能影响研究

Study on Effect of Compound Musk Injection Combined with Hyperbaric Oxygen on Brain Edema and Neurological Function in Patients with Medium-Volume Hypertensive Basal Ganglia Intracerebral Hemorrhage

目的:探讨复方麝香注射液联合高压氧对中等量高血压基底节区脑出血患者脑水肿及神经功能的影响。方法:将2016年1月-2018年1月本院中等量高血压基底节区脑出血患者192例,随机分两组,各96例,均采用微创颅内血肿清除术,在此基础上,对照组采用高压氧疗法治疗;观察组采用高压氧疗法联合复方麝香注射液10~20 mL治疗,比较两组患者临床疗效、美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)、Barthel指数量表(barthel index,BI)及脑水肿体积、血清水通道蛋白-4(aquaporin-4,AQP4)水平,记录不良反应。结果:对照组有效率81. 25%低于观察组91. 67%,差异具有统计学意义(P <0. 05)。治疗前两组患者NIHSS量表、BI指数及脑水肿体积、血清AQP4比较,差异无统计学意义(P> 0. 05)。治疗后对照组和观察组NIHSS量表为(11. 20±3. 25)(7. 35±3. 10),BI指数为(51. 32±8. 94)(60. 55±11. 34),脑水肿体积为(8. 51±4. 23) mL、(5. 65±3. 57) mL,血清AQP4为(0. 11±0. 07)U/mL、(0. 05±0. 04) U/mL,对照组和观察组上述指标比较,差异具有统计学意义(P <0. 05)。治疗期间未出现头晕、气压伤、氧中毒、胃肠道不适等不良反应。结论:复方麝香注射液联合高压氧对中等量高血压基底节区脑出血患者的疗效确切,可降低脑水肿和AQP4水平,改善生活质量和神经功能,且患者可耐受,值得推广。

Objective:To investigate the effects of Compound Musk Injection combined with hyperbaric oxygen on brain edema and neurological function in patients with moderate hypertensive intracerebral hemorrhage in basal ganglia.Methods:A total of 192 patients with moderate hypertensive intracerebral hemorrhage in basal ganglia from January 2016 to January 2018 were randomly divided into two groups,96 in each group.On this basis,the control group was treated with hyperbaric oxygen therapy and the observation group was treated with hyperbaric oxygen therapy combined with compound musk injection 10-20 mL,and the clinical efficacy of the two groups was compared with that of the National Health Research of the United States.National Institute of Health Stroke Scale(NIHSS),Barthel Index(BI),brain edema volume and serum aquaporin-4(AQP4)levels were recorded,and adverse reactions were recorded.Results:The effective rate of the control group was 81.25%,lower than that of the observation group(91.67%,P<0.05).There was no significant difference in NIHSS scale,BI index,brain edema volume or serum AQP4 between the two groups before treatment(P>0.05).After treatment,the NIHSS scale of the control group and the observation group were11.20±3.25 and 7.35±3.10.BI index of the control group and observation group was 51.32±8.94 and 60.55±11.34.The brain edema volume of the control group and observation group was(8.51±4.23)mL and(5.65±3.57)mL and serum AQP4 was(0.11±0.07)U/mL and(0.05±0.04)U/mL.The difference between the control group and the observation group was statistically significant(P<0.05).No adverse reactions such as dizziness,barotrauma,oxygen poisoning or gastrointestinal discomfort occurred during the treatment.Conclusion:Compound Musk Injection combined with hyperbaric oxygen has a definite effect on moderate hypertensive cerebral hemorrhage in basal ganglia region.It can reduce brain edema and AQP4 levels,improve quality of life and neurological function,and it is tolerable for patients.It is worthy of popularizing.
