穴位埋线治疗围绝经期轻中度抑郁的临床研究

Clinical Study on Treating Mild-to-Moderate Perimenopausal Depression with Catgut Implantation at Acupuncture Points

目的:通过观察穴位埋线与百忧解治疗围绝经轻中度抑郁的临床疗效,进一步验证穴位埋线的有效性和安全性。方法:将130例患者随机分为试验组和对照组,每组65例。试验组采用穴位埋线治疗,取穴:肾俞、肝俞、心俞、三阴交(均为双侧取穴),每2周1次。对照组:给予抗抑郁西药百忧解(盐酸氟西汀分散片)治疗,20 mg/d,晨起顿服,两组均治疗4周,随访4周。采用汉密顿抑郁量表(HAMD)和围绝经期症状量表(MRS)在治疗前后及随访第4周对两组患者进行抑郁症状评分,Asberg氏抗抑郁药不良反应量表(SERS)对两组副作用进行评分。结果:与治疗前相比,两组治疗4周、随访4周的HAMD总分和MRS评分均明显下降(P <0. 01),两组治疗前后临床疗效比较无明显差异(P> 0. 05),但随访4周试验组的临床疗效优于对照组,差异具有统计学意义(P <0. 05),两组HAMD评分各维度分值均较治疗前有显著降低(P <0. 01),在睡眠和焦虑/躯体化改善方面,试验组较对照组有显著性差异(P <0. 01)。试验组治疗后SERS评分较对照组低,差异具有统计学意义(均P <0. 05)。结论:穴位埋线是治疗围绝经轻中度抑郁的有效手段,较西药百忧解更安全且疗效更持久,能够对机体进行整体调节,不失为理想的治疗方法。

Objective:To assess the efficacy and safety of catgut implantation at acupuncture points in the treatment of mild-to-moderate perimenopausal depression.Methods:With a multi-center randomized controlled study,130 cases were randomly divided into a treatment group and a control group.The treatment group was treated with catgut implantation at acupuncture points at Shenshu(BL23),Ganshu(BL18),Xinshu(BL15)and Sanyinjiao(SP6).The control group was treated with fluoxetine 20 mg a day.Both groups were treated for 4 weeks and followed up for another 4 weeks.Hamilton Depression Scale(HAMD)and Menopause Rating Scale(MRS)were assessed at baseline and 4 weeks after treatment.Results:The total HAMD and MRS scores of baseline and 4 weeks treatment and 4 weeks follow-up were all reduced in both groups(all P<0.01).The total scores of 4 weeks follow-up in treatment group were lower than those in the control group with significant differences in statistical analysis(P<0.05).Compared with the clinical the therapeutic effect of both groups after 4 weeks treatment,there was no significant difference(P>0.05).The safety indexes before and after treatment of treatment group was lower than those of the control group with significant differences in statistical analysis(P<0.05).Conclusion:Catgut implantation at acupuncture points is an effective and safe method for menopausal depression.
