阿托伐他汀对心源性脑栓塞患者T淋巴细胞亚群的影响

Effect of atorvastatin on T-lymphocyte subsets in patients with cardiogenic cerebral embolism

目的通过研究心源性脑栓塞患者阿托伐他汀使用前后T淋巴细胞亚群及超敏C-反应蛋白(hs-CRP)的影响,了解他汀类药物对心源性脑栓塞的免疫和炎症方面的影响,以探讨他汀类药物治疗脑栓塞的可能机制。方法选择47例临床诊断为心源性脑栓塞患者,随机分成阿托伐他汀治疗组(25例)和对照组(22例),治疗组在脑梗死常规治疗基础上每晚口服阿托伐他汀钙胶囊20mg·d-1,对照组则仅采用脑梗死常规治疗,共12w,治疗前后测定两组外周血T淋巴细胞亚群及hs-CRP;并且根据患者治疗后神经功能缺损评分变化进行效果评价。结果两组治疗前外周血T淋巴细胞亚群及hs-CRP值无明显差别,治疗后治疗组CD4+T、CD4+/CD8+较治疗前明显升高,CD8+T、hs-CRP值较治疗前明显降低,且与对照组比较差异有统计学意义(P<0.05)。结论阿托伐他汀可调节心源性脑栓塞患者外周血T淋巴细胞活性,降低hs-CRP,且治疗效果明显好于对照组,能够有效改善症状。

Objective To study atorvastatin in patients with cardiogenic cerebral embolism before using T-lymphocyte subsets and high-sensitivity C-reactive protein( hs-CRP),and to understand statins on cardiovascular origin of cerebral embolism of immune and inflammatory effect,in order to investigate statin treatment possible mechanism for the treatment of cerebral embolism. Methods Chose 47 cases clinical diagnosis CCE,and divided them into treatment group( 25 cases) in often treatment on the basis of every night oral atorvastatin 20 mg and control group( 22 cases) with cerebral infarction conventional treatment,between pre-therapy and post-therapy. Tested the peripheral blood T-lymphocyte subsets and hs-CRP relative counting with flow cytoanalyze. And evaluate the effect of the therapy according to the degree of defect in nerous function.Results Two groups before treatment of peripheral blood T-lymphocyte subsets and hs-CRP value has no obvious difference,the treatment group after treatment CD4+T,CD4+/ CD8+before a treatment significantly increased,and CD8+T,hs-CRP value is significantly lower than before treatment,and compared with the control group with significant difference( P < 0. 05). Conclusion Atorvastatin adjustable cardiogenic cerebral embolism in patients with peripheral blood T-lymphocyte activity and reduces hs-CRP,good treatment effect,can effectively improve the symptoms.
