大黄、皮硝外敷改善转移性癌性骨痛临床研究

Clinical Study of Rhubarb and Mirabilite External Treatment on Improving Metastatic Cancer-induced Bone Pain

目的:评价大黄、皮硝联合西药改善转移性癌性骨痛的疗效及安全性。方法:对照组应用唑来膦酸针4 mg+100 mL生理盐水抗骨转移治疗,静脉滴注30 min以上,每4周治疗1次;同时给予口服盐酸羟考酮缓释片(奥施康定);治疗组在对照组治疗的基础上,患者每日配合大黄、皮硝(比例1∶4)外敷肿胀/疼痛最明显处。结果:比较治疗组与对照组总有效率的差异无统计学意义(P>0.05)。比较治疗组与对照组的止痛起效时间及止痛持续时间,差异较为明显(P<0.01)。生存质量症状评估中治疗组与对照组在与他人关系、一般情绪、睡眠不安、昏昏欲睡感、恶心、呕吐、生活情趣等方面的差异有统计学意义(P<0.05)。试验过程中未出现与试验相关的明显不良反应及肝肾功能损伤。结论:大黄、皮硝外敷联合西药治疗能够缩短止痛起效时间,延长止痛持续时间,从而达到加强止痛的作用,并能改善患者的部分生活质量,安全性高。

Objective:To evaluate the effect and security of rhubarb and mirabilite in combination with Western medicine to improve the metastatic cancer-induced bone pain. Methods:The control group used zoledronic acid 4 mg in the treatment of metastatic bone,above 30 min infusion treatment every 4 weeks;while giving oxycodone hydrochloride prolonged-release tablets(oxycontin);treatment group based on the treatment of patients in the control group with skin daily rhubarb and mirabilite(ratio 1∶4)at the place of pain/swelling obvious. Results:Comparing the difference between the treatment group and control group total effective rate,no statistical significance(P>0.05).To compare the treatment group and control group in the analgesic effect of time and duration of pain,with obvious difference(P<0.01). Symptoms of quality of life assessment of the treatment group and control group in relationship with others,general mood,sleep,sleepy feeling,nausea,vomiting,life interest difference was statistically significance(P<0.05). There was no obvious adverse reaction and liver and kidney function injury associated with the study. Conclusion:Rhubarb and mirabilite external treatment combined with Western medicine can shorten the onset time of analgesia and prolong the duration of pain relief,to strengthen the role of control pain. It can improve the patient’s life quality and have high safety.
