加味黄连温胆汤治疗痰热互结型代谢综合征合并H型高血压临床研究

Clinical Research of Modified Huanglian Wendan Decoction for the Treatment of Metabolic Syndrome in Combination with Phlegm,Heat and H-hypertension

目的:研究加味黄连温胆汤对痰热互结型代谢综合征(Metabolic syndrome,MS)合并H型高血压的改善作用。方法:选择48例符合痰热互结型MS合并H型高血压患者,随机分为两组,对照组予常规基础治疗,包括改善生活方式,针对高血压、高血糖、高血脂分别予降压、补充叶酸、降糖、调节血脂治疗,治疗组在对照组基础上加服加味黄连温胆汤,治疗4周后观察MS和H型高血压各组分指标。结果:两组患者治疗后中医证候积分较治疗前明显下降,且治疗组优于对照组(P<0.05);临床疗效方面,治疗组总有效率达87.5%,对照组总有效率70.8%,治疗组优于对照组(P<0.05);治疗后治疗组形体学指标BMI、WHR优于对照组,治疗组血压、血HCY水平、空腹血糖(FBG)、餐后2小时血糖(2 h BG)、糖化血红蛋白(HbAlc)、TG、TC、LDL-C水平明显降低,HDL-C水平升高,且明显优于对照组(P<0.05)。结论:加味黄连温胆汤联合常规治疗能够有效改善痰热互结型代谢综合征合并H型高血压患者中医证候,并改善其代谢异常状态。

Objective:To research the improvements of Modified Huanglian Wendan Decoction for the treatment of metabolic syndrome(MS)in combination with phlegm,heat and H-hypertension. Methods:Choose 48 patients who conform to the condition of metabolic syndrome(MS)in combination with phlegm,heat and H-hypertension and randomly divide the patients into 2 groups. The control group was treated with a regular basis,including lifestyle improvement,blood pressure control,folic acid supplement,blood sugar level and blood lipids regulation. Differently,the treatment group was given Modified Huanglian Wendan Decoction additionally. Both groups were treated for 4 weeks in order to observe index of MS and H-hypertension. Results:After two groups of treatment,TCM syndrome score was significantly lower for both groups,score of treatment group was better than the control group(P<0.05). As for clinical effects,the total effectiveness was 87.5% for treatment group,however,70.8% for the control group,in short,treatment group was better than control group(P<0.05). After different treatment,physique study index,BMI and WHR of treatment group were better than control group,moreover,index such as blood pressure,blood levels of HCY,FBG,2 h BG,HbA1 c,TG,TC and LDL-C level of treatment group were significantly decreased. In addition,HDL-C level is increased,and being obviously better than control group(P<0.05). Conclusion:Modified Huanglian WendanDecoction combined with regular treatment may effectively improve the traditional Chinese medicine syndrome of metabolic syndrome(MS)in combination with phlegm,heat and H-hypertension,and improve the status of metabolic abnormalities.
