中药熏蒸治疗癌性疼痛伴焦虑抑郁状态患者临床观察

Clinical Observation of TCM Fumigation Nursing in Treating Cancer Pain with Anxiety and Depression

目的:探讨中药熏蒸治疗癌性疼痛伴焦虑抑郁状态患者的临床疗效。方法:选取我院肿瘤科收治的癌症患者共76例,随机分为对照组和治疗组,各38例。其中对照组予以癌痛常规西医治疗;治疗组在对照组的基础上给予中药熏蒸治疗,两组患者均治疗4周。比较两组患者癌痛缓解率、疼痛视觉模拟评分(VAS)、抑郁自评量表(SDS)评分及焦虑自评量表(SAS)评分、止痛起效时间、止痛持续时间、爆发痛次数及生存质量改善情况。结果:经治疗,治疗组患者癌痛缓解率为84.21%,对照组患者癌痛缓解率为68.42%,治疗组显著优于对照组,差异具有统计学意义(P<0.05);治疗后,治疗组患者每日药效止痛起效时间、止痛持续时间及睡眠时间均显著优于对照组,差异具有统计学意义(P<0.05);与对照组相比,治疗组患者VAS评分、SDS评分及SAS评分改善更显著(P<0.05);治疗组患者生理领域、心理领域、社会领域及环境领域生存质量评分较对照组降低程度更明显(P<0.05)。结论:中药熏蒸疗法可起到通经活络、行气止痛的临床疗效,有效降低患者疼痛程度,改善患者抑郁、焦虑、烦躁等不良情绪,且本方案毒副作用小,安全系数高,易于患者接受,值得临床推广应用。

Objective:The purpose of this study was to investigate the effect of TCM fumigation nursing on cancer pain with anxiety and depression.Methods:76 cancer patients admitted to our hospital oncology department were selected and randomly divided into treatment group and control group with 38 cases in each group.The control group was given conventional Western medicine treatment for cancer pain.The treatment group was treated with TCM fumigation nursing therapy on the basis of the control group.The patients in both groups were treated for 4 weeks.The cancer pain relief rate,the Visual Analogue Scale(VAS),Self-rating Depression Scale(SDS),Self-Rating Anxiety Scale(SAS),time of onset of pain relief,duration of pain relief,number of outbreaks of pain,and improvement of life quality of the patients in the two groups were compared.Results:After treatment,the cancer pain relief rate in the treatment group and control group were 84.21%and 68.42%,respectively,and the cancer pain relief rate of the treatment group was significantly better than that of the control group,the difference was statistically significant(P<0.05).The time of onset of daily pain relief,duration of pain relief,and sleep time in the treatment group were significantly better than that of the control group after treatment,and the difference was statistically significant(P<0.05).The VAS score,SDS score and SAS score improved more significantly in the treatment group than that in the control group(P<0.05).The reduction of patients’life-quality scores in physiological,psychological,social and environmental fields were even greater in the treatment group than in the control group(P<0.05).Conclusion:Traditional Chinese medicine fumigation can play a clinical role in dredging meridian passage,activating Qi and relieving pain,effectively reducing the pain level of patients and improving their bad mood such as depression,anxiety and dysphoria.The scheme has the advantages of small toxic and few side effects,high safety factor and easy acceptance by patients,and is worthy of clinical popularization and application.
