兰州市不良妊娠结局疾病负担现状研究

The economic burden of common adverse pregnancy outcomes in Lanzhou City, China

目的计算不良妊娠结局的住院费用,为减轻不良妊娠结局经济负担提供基础数据。方法调取兰州市妇幼保健院新生儿住院病历资料,应用描述性研究方法,计算不良妊娠结局新生儿的住院费用和单项费用构成。结果兰州市妇幼保健院常见不良妊娠结局的例均费用为2 154.83元,早产儿、出生缺陷、低体重儿和新生儿窒息的例均费用分别为2 173.27、2 188.70、1 908.48、2 275.18元,差异不具有统计学意义(P> 0.05)。自费占支付方式的71.38%,住院费用中治疗费和护理费分别占26.98%、23.33%,而出生缺陷中护理费和化验费分别占26.27%、17.93%。结论不良妊娠结局各病种的住院费用没有显著性差异(P> 0.05),但以新生儿窒息为最高;自费仍是其主要支付方式;早产儿、低出生体重儿和新生儿窒息以治疗费和护理费为主,出生缺陷中护理费和化验费占比较高。

Objective To calculate the hospitalization expenses of adverse pregnancy outcomes, provide the basic data for reducing the economic burden of thereof. Methods Descriptive research methods were used to calculate the hospitalization expenses of neonates with adverse pregnancy outcomes, according to the data of neonatal inpatient records of Lanzhou City Maternity and Child-care Hospital. Results The average cost of each case with common adverse pregnancy outcomes in was 2 154.83 yuan. The average cost for preterm infant, birth defect, low birth weight infant, and neonatal asphyxia was 1 173.27, 2 188.70, 1 908.48 and2 275.18 yuan respectively. The differences were not statistically significant(P > 0.05). Self-paid expenses accounted for 71.38% of payment. Of the hospitalization expenses, treatment and nursing costs accounted for26.98% and 23.33%, while nursing and laboratory tests for birth defects accounted for 26.27% and17.93% respectively. Conclusion No significant difference has been found in the cost of hospitalization for several diseases of adverse pregnancy outcomes(P > 0.05), but neonatal asphyxia was the highest. Self-paid expenses are still the main way of payment. The hospitalization expenses for preterm infants, low birth weight infants and neonatal asphyxia were mainly used for treatment care and nursing fees, while nursing and tests fees accounted for higher in birth defects.
