产前血型抗体水平监测及其临床意义

Determination of Level of Biood Type Antibody and its Clinical Significance Before Delivery

目的 检测血型不合夫妇的产前血型IgG抗体水平 ,探讨预防及诊治新生儿溶血病 (IIDN)的有效方法。方法 选择夫妇ABO血型不合者 4 91例 ,采用血型血清学方法进行相关检测及归纳分析。结果 夫妇ABO血型不合发生率为 35 .4 %。 4 91例ABO血型不合调查中 ,IgG抗A(B)效价 <6 4占 2 7.6 0 %效价≥ 6 4占 4 8.9% ,效价≥ 2 5 6占 2 3.5 %。孕产史与IgG抗体水平成正比。妊娠中IgG抗体变化与胎儿血型密切相关。相关调查结果与临床追踪新生儿溶血病的发生相符。结论 夫妇血型不合应及时检测产前血型抗体 ,可预报HDΝ发生及胎儿受害程度 。

Objective To derect the level of IgG antibody of the parents whose blood tyes are incompatibe betore delivery,and to discuss the effective ways to the prevention of hemclytic disease of the newborn(HDN) and its diagnosis and therapy.Methods Relative and deductve analysis were carried out on 491 female patients who had blood type incompatibility.Results The incidence of ABO blood type incompatibility between conjugal ties was 35.4%.In 491 patients of ABO blood type incompatibility.titers of JgG antibody lowere than 64 were found in 27% of the paticnts.Titcrs of IgG antibody cqual to or more than 64 wcrc found in 48,9%,and titers of IgG antibody cqual to or morc than 256 found in 23.5%.The changc of IgG antibody in gestation was relative to fetus blood type.The investigation was accordant with incidence of HDN achievrd through follow up.Conclusion It is suggesed that blood type and antibody be tested on those with blood type incompatibility between conjugai ties befors delivery so that HDN and the damage to fetus could be forecasl.The finding ahove acts as a guide to clinical diagnosis and therapy.
