猪多器官功能障碍综合征合并急性肾衰竭模型的制作 被引量:8

A Study of Pig Model of Multiple Organ Dysfunction Syndrome with Acute Renal Failure

摘要 目的:建立一种多器官功能障碍综合征(MODS)合并急性肾衰竭(ARF)的动物模型。方法:12只健康杂交家猪随机分成实验组(E组)和假手术对照组(C组)。E组行盲肠结扎加穿孔(CLP),并加做双侧输尿管结扎术(UL)制作成MODS合并ARF的动物模型,C组行盲肠和双侧输尿管探查术。两组动物均予24 h静脉输液,观察6天中动物的生命体征及主要器官功能改变。结果:E组动物在CLP+UL后4 h直肠温度升高;术后72 h血清肌酐(Cr)、尿素氮(BUN)、K+、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)水平分别从术前的(91.40±10.85))μmol/L、(4.08±0.93)mmol/L、(3.81±0.57)mmol/L、(62.40±8.20)U/L、(53.00±14.85)U/L升高至(995.25±125.28)μmol/L、(48.80±1.41)mmol/L、(7.37±0.66)mmol/L、(115.75±33.42)U/L、(278.00±96.20)U/L,动脉血二氧化碳分压(PaCO2)从术前的(37.68±3.10)mmHg(1mmHg=0.133 kPa)升高至(51.98±4.51)mmHg(P均<0.01);pH值分别从术前的7.38±0.04下降至7.25±0.05(P<0.05);平均动脉压(MAP)从术后0 h的(160.20±18.58)mmHg降至术后72 h的(105.75±13.96)mmHg(P<0.01),6天内动物全部死亡,平均生存时间(74.96±23.00)h。C组全部存活。且上述指标基本正常。结论:盲肠结扎加穿孔造成严重腹腔感染致明显的多器官功能障? Objective: To establish a composite pig model with multiple organ dysfunction syndrome (MODS) and acute renal failure (ARF). Methods: 12 pigs were randomly divided into two groups, group E received cecal ligation and puncture (CLP) plus bilateral ureteral ligation and group C received sham operation. The animals were infused with solutions containing glucose and sodium chloride for 6 days. Results: In group E, the rectal temperature increased 4 hours after operation. The indexes enhanced 72 hours after operation, including SCr from (91.40 ± 10. 85) μmol/L to (995. 25 ±125. 28)μmol/L, BUN from (4. 08 ± 0. 93) mmol/L to (48. 80±1.4l)mmol/L, serum kalium from (3.81 ±0.57)mmol/ L to (7. 37±0. 66)mmol/L, ALT from (62. 40 ±8. 20)U/L to (115. 75± 33. 42)U/L, AST from (53. 00 ±14. 85)U/L to (278. 00±96. 20)U/L and PaCO2 from (37. 68±3. 10)mmHg to (51.98±4.5l) mmHg ( P < 0. 01). The blood value of pH decrease from 7. 38±0. 04 to 7. 25±0. 05, ( P < 0. 05), and the value of MAP reduced from (160. 20 ± 18. 58) mmHg to (105. 75 ±13. 96)mmHg( P <0.01). The mortality in group E is 100% within 6 days after operation, and the survival time was (74. 96 ±23. 00)h. All the animals, survived in group C. Conclusion: The dysfunction in organs and systems was caused by severe intra-abdominal infection induced by CLP, meanwhile, acute anuric renal failure was produced by bilateral ureteral ligation. It was successful to produce the pig model with MODS and ARF.
