中心性渗出性脉络膜视网膜病变的经瞳孔温热治疗

Transpupillary thermotherapy for central exudative chorioretinopathy

目的 评价经瞳孔温热疗法 (transpupillary thermotherapy,TTT)治疗中心性渗出性脉络膜视网膜病变 (central exudative chorioretinopathy,CEC)的疗效。方法 对 12例 (12只眼 )黄斑区具有脉络膜新生血管(choroidal neovascularization,CNV)的中心性渗出性脉络膜视网膜病变患者进行 TTT治疗 ,并随访观察治疗前后视力、眼底、眼底荧光血管造影 (fundus fluorescein angiography,FFA)及吲哚青绿眼底血管造影 (indocyanine greenangiography,ICGA )的变化。结果 12只眼中有 3只眼进行了二次治疗 ,最终所有患者眼底出血渗出的情况均得到改善 ,眼底造影显示 CNV缩小或闭塞 ,治疗后视力提高 3只眼 ,稳定 6只眼 ,下降 3只眼。结论 经瞳孔温热疗法可以减少因脉络膜新生血管膜而引起的出血及渗出 ,加速疤痕化 ,对黄斑区 CNV有一定的疗效 。

Objective To evaluate the efficacy of transpupillary thermotherapy(TTT)for central exudative chorioretinopathy (CEC).Methods Transpupillary thermotherapy was performed on 12 eyes of 12 patients who suffered from CNV of CEC.Visual acuity,Fundus fluorescein Angiography (FFA) and Indocyanine Green Angiography (ICGA) were observed before and after treatment.Results Among 12 eyes,3 eyes underwent second treatment.All the hemorrhage and exudation were absorbed and angiography showed occlusion of CNV.Visual acuity were improved in 3 eyes,remained stable in 6 eyes and declined in 3 eyes after treatment.Conclusion TTT can reduce exudation and hemorrhage caused by CNV,results in faster cicatrization of the CNV complex and proved to be an acceptable therapy.But the improvedment of center visual acuity is optimal.
