特拉唑嗪和卡托普利对慢性充血性心力衰竭病人左室重塑的影响对比

Effects of Terazosin and Captopril on The Left Ventricular Remodeling of SCHF

目的 对比α受体拮抗剂和血管紧张素转换酶抑制剂 (ACEI)对慢性充血性心力衰竭病人 (SCHF)心室重塑的影响。方法将CHF165人随机分为对照组 5 5例、特拉唑嗪组 5 5例和依拉普利组 5 5例 ,于治疗前后进行UCG检查 ,测定心率 (P)、平均动脉压(MAP)、心排量 (SV)、左室射血分数 (LVEF)、心输出量 (CO)、心脏指数 (CI)、室间隔厚度 (LVST)、左室后壁厚度 (LVPWT)、左室舒张末内径 (LVDd) ,计算左室心肌重量指数 (LVMI) ,了解二者对心室重塑的阻抑作用。结果 特拉唑嗪组、卡托普利组与对照组比较 ,除了心率、平均动脉压无显著性差异外 ,SV、LVEF、CO、CI均明显升高 (P <0 .0 5 ) ,LVPWT、IVST、LVDd、LVMI均明显降低 (P <0 .0 5 ) ,而特拉唑嗪组和卡托普利组的各参数 ,在治疗前后无显著性差异。结论 α受体拮抗剂能抑制慢性充血性心力衰竭病人的左室重塑过程 ,改善左室功能 。

Objective To compare the inhibitory effects of α antagonist Terazosin versus angiotensin converting enzyme inhititors(ACEI) Captopril on left ventricular remodeling(LVRM)of Slow aACongestive Heart Failure(SCHF).Methods A total of 165 patients with SCHF were entered into the study.55 patients were randomized to conventional treatment group,55 patients to Terazosin treatment group and 55 patients to Captopril treatment group.At the beginning and end of the treatment,P?MAP?SV?LVEF?CO?CI?LVST?LVPWT?LVDd and LVMI were mensured by echocardiogram.Results Campared Terazosin treatment group?Captopril treatment groups with conventional treatment groups,SV?LVEF?CO?CI were significantly increased,IVST?LVPWT?LVDd and LVMI were significantly decreased( P <0.05),except P?MAP( P >0.05).And there was no difference in all of the parameters between the Terazosin treatment group and Captopril treatment groups.Conclusionα antagonist can inhibit LVRM of SCHF,improve the left ventricular systolic synchrony,while the inhibitory effect on LVRM of Terazosin was similar to Captopril.
