地塞米松和甲基泼尼松龙对哮喘儿童外周血Th1/Th2类细胞因子平衡的影响 被引量:28

摘要 目的 观察支气管哮喘儿童外周血单个核细胞 (PBMC)中CD4 +T细胞中两个功能性亚群 (Th1/Th2 )的功能状态及地塞米松和甲基泼尼松龙对其的影响。方法  15例哮喘及 14名健康儿童 ,取静脉血 ,密度梯度离心法分离PBMC ,分别用 10 -8mol/L地塞米松和甲基泼尼松龙体外干预培养 4 8h后 ,用ELISA法测定培养上清中的干扰素 γ(IFN γ)、白细胞介素 4 (IL 4 )和IL 10的浓度。结果 地塞米松和甲基泼尼松龙均可明显抑制哮喘儿童PBMC分泌IFN γ(P值分别为 0 .0 0 4和 0 .0 0 2 )和IL 4(P值分别为 0 .0 0 2和 0 .0 0 1) ;若以IFN γ/IL 4表示Th1/Th2间的平衡 ,则甲基泼尼松龙 (P <0 .0 5 )可恢复Th1/Th2平衡 ;地塞米松和甲基泼尼松龙都不影响IL 10的分泌。结论 地塞米松和甲基泼尼松龙均可抑制IFN γ和IL 4的产生 ,但甲基泼尼松龙抑制作用更强 。 Objective To observe the state of Th1/Th2 cytokine expression in peripheral blood mononuclear cells ( PBMCs ) from asthmatic children pas and the effects of dexamethasone ( DEX ) and methylprednisolone ( MP ) on it. Methods 15 asthmatic children and 14 healthy volunteers were included. PBMCs were isolated from venous blood using Ficoll Hypaque density gradient centrifugation. 10 -8 mol/L DEX or MP was added to cultures at the time of initial stimulation. After PBMCs were incubated for 48h, supernatant was harvested. Cytokine levels of interferon gamma, IL 4 and IL 10 in culture supernatants were determined by ELISA. Results DEX and MP can inhibit obviously the secretion of IFN γ( P =0.004, 0.002 respectively) and IL 4( P =0.002, 0.001 respectively ); If the balance of Th1/Th2 is represent by IFN γ/IL 4, MP can restore the imbalance ( P = 0.048 ); DEX and MP do not effect the production of IL 10. Conclusion DEX and MP can all inhibit the secretion of IFN γand IL 4, besides, MP can restore the imbalance of Th1/Th2 existed in asthmatic children.
