颈前路带锁钢板治疗下颈椎损伤(附38例报告)

Application of anterior cervical plate in the treatment of lower cervical injury (38 cases report)

目的 研究颈前路带锁钢板在下颈椎损伤手术中的应用。方法 2 0 0 0年 4月 - 2 0 0 2年4月止采用颈椎前路减压、植骨融合、带锁钢板内固定治疗下颈椎骨折脱位并颈髓损伤 3 8例 ,其中单节段 2 2例 ,双节段 12例 ,三节段 4例 ;Frankel分级 :A级 4例 ,B级 12例 ,C级 16例 ,D级 6例。结果 术后随访 6～ 12月 ,平均 10 .5月 ,3 8例植骨块于 3月内骨性愈合 ,平均愈合时间为 2 .6月 ,脊髓功能均有不同程度恢复 ,Frankel分级平均提高 1.4级 ,无 1例有骨不连、植骨块脱出、钢板螺钉松动脱出等并发症。结论 颈前路带锁钢板是颈椎前路减压、植骨融合手术中较为安全。

Objective To study the application of anterior cervical plate in the operation of lower cervical injury.Methods From Apirl,2000 to Apirl,2002, 38 patients with lower cervical facture or dislocation complicated by spinal cord injury were treated by anteroir decompression ,fusion and internal fixation with locking plate. Among them, there were 22 with one segment injury, 12 with two,4 with three; according to Frankel grade,4 were A,12 B,16 C and 6 D.Results All patients were followed-up for 6～12 months (mean,10.5 months). All the bone grafts fused in three months (mean,2.6 months). Spinal functions of the patients recovered in different degrees,meanly raised 1.4 Frankel grades.There was no unfusion,migration of the grafts,loose and migration of the plates.Conclusion The anterior cervical plate is a safer,more effective internal fixation system in the operation of anterior decompression and fusion.
