摘要
目的探讨STI571治疗慢性髓系白血病的毒副反应谱、发生率及相应的护理方法。方法对接受STI571治疗的慢性髓系白血病22例(慢性期6期,加速急变期16例)患者,治疗期间密切观察各系统发生的毒副反应并作出相应的护理。毒副反应分级参考美国癌症研究所常见毒性反应标准。统计毒副反应的种类及发生率,并对慢性期与加速/急变期的造血系统毒副反应的发生率进行比较。结果22例患者均出现了Ⅰ/Ⅱ级非造血系统毒副反应,常见的有各部位水肿(77%)、消化系统反应(36%)和肌肉疼痛(23%)等。严重的造血系统毒副反应包括Ⅲ/Ⅳ级中性粒细胞减低9例,其中慢性期患者为1例,加速/急变期患者8例;Ⅲ/Ⅳ级血小板减低6例,均为加速/急变期患者。慢性期与加速/急变期患者Ⅲ/Ⅳ级中性粒细胞/血小板减少的发生率均无统计学差异。结论STI571治疗慢性髓系白血病发生的非造血系统毒副反应较轻,有较好的安全性;造血系统反应中Ⅲ/Ⅳ中性粒细胞减少和血小板减少在加速/急变期的发生率较在慢性期有更高的趋势。
Obje ct ive To explore the sidereaction profile and occurrence rate of STI571in the trea tment of chronic myeloid leukemia and the relative nursing methods.Methods22case s of CML,including6cases in chronic phase and16cases in ac-celerated/blast cri sis phase,were observed and the side reaction and adverse events were evaluated .Toxicity and side reaction were graded in accordance with the Common Toxicity C riteria of the National Cancer Institute.The percentage of hematological toxicit y in chronic phase and accelerated/blast crisis phase was compared.Res ultsⅠ/Ⅱ grade non-hematological toxicity was observed in all the patients,including edem a(77%),side reaction in digestive system(36%)and myalgia.Severe hematological to xicity includ-edⅢ/Ⅳgrade neutropenia(9cases)and thrombocytopenia(6cases).The percentage ofⅢ/Ⅳneutropenia/thrombocytopenia in chron-ic phase and accelerate d/blast crisis phase was compared and no significant difference was observed sta tisticatly.Conclus ions Non-hematological toxicity of STI571in the treatment of chronic myeloid leukemia is low,which suggests this drug is re-lafively safe.Se vere hematological toxicity,such asⅢ/Ⅳneutropenia and thrombocytopenia,are mor e common in accelerated/blast crisis than in chronic phase.
