二甲双胍治疗多囊卵巢综合征胰岛素抵抗的临床观察

A Clinical Study:Metformin Treats the Insulin Resistance in Patients with Polycystic Ovarian Syndrome(PCOS)

目的 观察二甲双胍治疗多囊卵巢综合征胰岛素抵抗患者的临床疗效。方法 对 2 0例多囊卵巢综合征胰岛素抵抗患者 ,进行 12周的二甲双胍治疗 ,比较治疗前后的体重指数、基础胰岛素、生殖激素水平与排卵率的变化。结果 治疗 12周后 ,基础胰岛素水平从 19.71± 6 .2 7mU/L降至 14 .95± 5 .4 3mU/L(P <0 .0 2 ) ,LH从 18.2 0± 6 .91mU/L降至 13.17± 3.75mU/L(P <0 .0 1) ,睾酮从 1.89± 0 .4 3nmol/L降至 1.5 3± 0 .39nmol/L(P <0 .0 2 )。 2 0例月经稀发的患者 ,16例恢复排卵 ,排卵率 5 3%。结论 二甲双胍可改善胰岛素敏感性 ,降低血胰岛素水平 ,进而纠正了高雄激素血症等内分泌紊乱 。

Objective To observe the clinical effects of the metformin on patients with PCOS. Methods We have treated 20 cases of PCOS patients for consecutive 12 weeks and compared the body mass index (BMI) , basal insulin, reproductive hormones levels and ovultion rate before and after the treatment. Results After the treatment, basal insulin level have decreased from 19.71±6.27mU/L to 14.95±5.43mU/L( P <0.02,LH levels have decreased from 18.20±6.91mU/L to 13.17± 3.75 mU/L( P <0.01), and testerone levels have decreased from 1.89±0.43nmol/L to 1.53± 0.39 nmol/L( P <0.02). 16 cases of patients have ovulated spontaneously in 20 oligomennorrhea patients , accounting for 53%. Conclusion Metformin corrects the endocrinal disturbances to make patients ovulate through the improved insulin sensitivity and decreased blood insulin level.
