摘要
目的 探讨 β型趋化因子MCP -1(monocyteschemotacticprotein -1,MCP -1)在充血性心力衰竭发病中的作用。 方法 用RT -PCR检测 2 5例充血性心力衰竭患者和 10例正常人外周血单个核细胞MCP -1的表达 ,并用Bio -Rad图像分析系统对其进行相对定量 ,相关分析法确定MCP -1与充血性心力衰竭各临床指标之间的关系。结果 心力衰竭患者MCP -1的表达水平与正常对照组相比显著上调 (P <0 0 1) ;源于高血压、冠心病和扩心病的心力衰竭患者MCP -1表达水平与对照组相比均有显著性差异(P <0 0 1) ;扩张型心肌病心力衰竭患者MCP -1的表达水平显著高于高血压性心力衰竭组 (P <0 0 5 )。MCP -1的表达水平与外周血白细胞计数呈正相关 (γ =0 473,P <0 0 5 ) ,与淋巴细胞计数呈正相关 (γ =0 42 5 ,P <0 0 5 ) ,而与心力衰竭患者的射血分数呈负相关 (γ =-0 64 8,P <0 0 5 )。结论 充血性心力衰竭患者外周血趋化因子MCP -1表达上调可能是心力衰竭发生的病理生理因素之一 。
Objective To explore the role of β chemotactic factor- MCP-1 in the pathogenesis of congestive heart failure by detecting its expression changes.Methods PBMCs were collected from 25 patients with congestive heart failure and 10 healthy controls. RT-PCR was used to detect the mRNA expression of MCP-1 in PBMCs. Correlation of the clinical indexes of heart failure patients with to their MCP-1 mRNA expression levels were analysed.Results The expression level of MCP-1 in the heart failure patients was higher significantly than that of controls (P<0 001). In the heart failure patients from hypertension, or coronary heart disease, or dilated cardiomyopathy,the MCP-1 expression level was significantly higher than that of controls (P<0 01);The MCP-1 expression level of the patients with dilated cardiomyopathy was higher significantly than that of hypertension patients (P<0 05). Correlation analysis showed that the expression level of MCP-1 was correlated with not only white blood cell count (γ=0 473,P<0 05), but also lymphocyte count (γ=0 425,P<0 05), and it was significantly inversely correlated with ejection faction (EF) as well.(γ=-0 648,P<0 05).Conclusions The up-regulation of the expression of chemotactic factor MCP-1 in PBMC may be one of the pathophysiological factors in development and attack of congestive heart failure.Its mechasism may be related to the activation of the white blood cells.
