摘要
目的 :了解血管性痴呆 (VD)患者血浆、脑脊液 (CSF)中精氨酸加压素 (AVP)含量变化及川芎嗪干预治疗的疗效 ,并探讨其可能作用机制。方法 :VD组 5 0例 ,对照组 (中枢神经系统非器质性疾病 ) 2 6例。VD组采用川芎嗪 (2 6例 )治疗 ,以金纳多 (2 4例 )作对照。运用简易精神状态量表 (MMSE)、P30 0潜伏期 (P30 0PL)、Hachinski缺血量表 (HIS)、社会功能活动调查 (FAQ)评定其认知功能 ,用放射免疫分析法 (RIA)测定血浆、CSF中AVP含量。结果 :VD组血浆中AVP含量无显著变化 (P >0 .0 5 ) ,CSF中AVP含量显著降低 (P <0 .0 1) ,P30 0PL显著延长 (P <0 .0 1) ,MMSE评分显著下降(P <0 .0 1)。同治疗前比较 ,川芎嗪组、金纳多组患者血浆AVP含量无显著差异 (P >0 .0 5 ) ,CSF中AVP含量明显增高(P <0 .0 5 ) ,MMSE评分明显增高 ,(P <0 .0 5 ) ,P30 0PL显著缩短 (P <0 .0 1) ;但两组治疗后各指标相比无显著差异。 (P>0 .0 5 )。结论 :脑内AVP在VD的发病中可能起重要作用 ;川芎嗪治疗对VD有一定的改善作用 。
Objective: To study the relationship between the activity changes of arginine vasopressin (AVP) in plasma, cerebrospinal fluid (CSF) of patients with vascular dementia (VD) and its pathophysiology. and to explore the therapeutic effect of Tetramethylpyrazine (TMP) on VD and its mechanism. Methods: VD group (50 patients) was treated with TMP (26 cases) or Ginaton (24 cases). There were 26 patients in the control group. The cognitive function was evaluated by Mini Mental State Examination (MMSE), P 300 peak latencies (P 300 PL). Hachinski Ischemic Scale (HIS) Functional Activities Questionnaire (FAQ). The concentrations of AVP in plasma and CSF were measured by radioimmunoassay (RIA) technology in 50 patients of VD group, and 26 cases of the control group. Results: As compared with the control group, VD group has no significantly difference in the concentrations of AVP ( P > 0.05 ); AVP levels in CSF were markedly decreased ( P < 0.01 ), the scores of MMSE was significantly lowered ( P < 0.01 ),P 300 PL was markedly prolonged ( P < 0.01 ). In VD group, after being treated with TMP or Ginaton, level of AVP in CSF or scores of MMSE was significantly increased ( P < 0.05 ),P 300 PL was markedly shortened ( P < 0.01 ). After therapy no significantly difference was found in that between TMP group and Ginaton group ( P > 0.05 ). Conclusion: It plays an important role of activity changes of AVP in pathophysiology of VD. The one of mechanisms of TMP for VD may be related with regulating disfunctions of AVP in central nervous system.
