摘要
目的 了解人工耳蜗植入前配戴助听器对人工耳蜗植入后康复的作用。方法 选取人工耳蜗植入后进行康复训练的 5 8例患者 ,其中男 36例 ,女 2 2例 ,年龄为 2 .5~ 3岁。按照术前配戴助听器的时间不同 ,分为A、B、C 3组 ,A组为 12例没有配戴过助听器的患者 ,B组为 15例配戴了 2~ 4个月助听器的患者 ,C组为 31例配戴了 10~ 13个月助听器的患者 ,分别测试植入并开机训练后 1周、1个月、3个月的补偿听阈、Ling’s 6音、听音辨图等项目。结果 开机后 1周和 1个月时 3组的补偿听阈之间差异均无显著性 (P >0 .0 5 ) ,B、C组在开机 3个月时的补偿听阈要比A组好 (P <0 .0 5 ) ;开机 1周和开机 1个月时B、C组Ling’s 6音的识别率要比A组明显高 (P <0 .0 1) ,且C组的识别率也要比B组明显高 (P <0 .0 1) ,而在开机 3个月时3组的识别率无明显差异 ;A、B、C 3组在同期的听音辨图的能力依次增高 (P <0 .0 1)。结论人工耳蜗术前配戴助听器比未配戴助听器的效果好 ,术前配戴助听器 10~ 13个月的比配戴了 2~
Objective To explore the influence of hearing aid fitting before artificial cochlear implantation (ACI) on the outcome of post-operative rehabilitation in children. Methods Fifty-eight children(36 male and 22 females)aged 2.5 to 3.0 years and implanted with artificial cochlear were recruited and divided into 3 groups: Group A(n=12) who had never been fitted the hearing aid before ACI , Group B(n=15) received hearing aid for 2~4 months before ACI, and Group C(n=31) for 10~30 months before ACI. The aided hearing threshold as well as the Ling's 6 sounds tests were administered to all the children after they were implanted with the artificial cochlear and trained for 1 week, 1 and 3 months, respectively. Results There was no significant difference among the 3 groups, in terms of the aided hearing threshold, at 1 week and 1 month after the implantation (P>0.05). The Ling' s 6 sounds recognizing scores of groups B and C were significantly higher than those of group A (P<0.05) at 1 week and 1 month after switch-on of the implants, with group B, significantly higher than group C(P<0.01). Conclusion Beneficial effect can be expected if hearing aid fitting is given to children before the artificial cochlear implantation.
