镁离子对脑弥漫性轴索损伤治疗作用的临床研究

摘要 目的 探讨镁离子对脑弥漫轴索损伤 (DAI)的治疗作用。方法  5 8例 DAI患者随机分为试验组和对照组。试验组患者入院后 15 min内静脉注射硫酸镁 16 mmol,2 4 h内再静脉滴注硫酸镁 6 5 mmol;对照组患者不用此药。 1周后测血清髓鞘碱性蛋白 (MBP) ,2周后行格拉斯哥昏迷评分 (GCS) ,3个月后进行格拉斯哥治疗结果分级 (GOS)评定。结果 试验组和对照组的血清 MBP分别为 10 .7ng/ml± 4 .2 ng/ml和 13.8ng/m l± 3.7ng/ml,GCS分别为 8.6± 2 .6和 7.1± 3.2 ,GOS分别为 3.4± 1.6和 2 .8± 1.9(x± s) ,两组之间的血清 MBP和 GCS的差异在统计学上有显著性意义(P<0 .0 5 )。结论 镁离子对 DAI患者有显著的近期治疗效果。 Objective To investigate the therapeutic effect of magnesium ion on patients with diffuse axonal injury.Method 58 patients were randomly divided into experimental group and control group.30 patients in experimental group received 16 mmol /L magnesium sulfate within 15 mins and another 65 mmol/L within 24 hours after admission.28 patients in control group didnot receive magnesium sulfate. Serum myelin basic protein (MBP),Glascow coma score(GCS) and Glascow outcome score (GOS) in all patients were measured after 7 days,2 weeks and 3 months respectively and the data were analyzed statistically.Results The serum MBP,GCS and GOS of experimental group were 10.7 ng/ml±4.2 ng/ml,8.6±2.6 and 3.4±1.6 (±s) respectively, while those of control group were 13.8 ng/ml±3.7 ng/ml,7.1±3.2 and 2.8±1.9(±s) respectively. Between two group the serum MBP and GCS were differently significant (P<0.05),while the GOS was not (P>0.05).Conclusion Magnesium ion was effective in treatment of patients with diffuse axonnal injury at least in the near future.
