光学相干断层扫描血管成像在评估无视网膜病变糖尿病患者中的应用 被引量:3

The application of optical coherence tomography angiography in diabetic patients without diabetic retinopathy

摘要 目的比较糖尿病无视网膜病变患者与正常人黄斑区微血管及视网膜神经纤维层(RNFL)厚度改变及其临床意义。方法收集2018年1月~2018年6月于潍坊医学院附属医院眼科中心就诊的2型糖尿病无视网膜病变患者43例55眼为NDR组,另外选择年龄相匹配的健康查体的正常健康者26例40眼为对照组。所有入选研究对象均采用光学相干断层扫描血管成像(OCTA)的Angio Retina 3.0mm模式扫描以中心凹为中心3mm×3mm范围;以ONH模式扫描视盘周围视网膜神经纤维层厚度,观察两组患者黄斑中心凹无血管区(FAZ)面积、黄斑区血流密度(MVD),测量浅层毛细血管丛(SCP)、深层毛细血管丛(DCP)血流密度及RNFL厚度的变化。结果 NDR组FAZ面积、SCP,DCP血流密度、RNFL分别与对照组比较,差异皆有统计学意义(P<0.05)。结论 NDR组较对照组FAZ面积扩大,SCP,DCP血流密度减低,RNFL变薄。 Objective To observe the changes in the area of macular and retinal nerve fiber layer(RNFL)in non-diabetic retinopathy(NDR)and normal control group using optical coherence tomography kangiography(OCTA)and its clinical significance.Methods Forty-three patients with type 2 diabetes were enrolled in The affiliated hospital of weifang medical university from January to June in 2018.including 55 eyes(43 patients)with NDR.Additional 40(26 patients)normal eyes with matched age are enrolled in the control group.All the patients have underwent the 3 mm×3 mm scanning of macular retina by optical coherence tomography angiography,and get the foveal avascular zone(FAZ)and macular vascular density(including SCP,DCP).To observe the thickness changes of RNFL around optic disc in 2 groups by OCTA.Results Compared with the control group,the area of FAZ,SCP,DCP,RNFL of the NDR group was statistically significant(P<0.05).Conclusion In diabetic patients without diabetic retinopathy,the area of FAZ is obviously enlarged than that in normal people.The patient of diabetic have decreased SCP,DCP blood flow density in diabetics without diabetic retinopathy and the thickness of the RNFL became thinner.
