人工晶体瞳孔夹持的分析

Analysis of intraocular lens pupillary capture

目的 分析后房型人工晶体植入术后发生瞳孔夹持的原因 ,并探讨其防治方法。方法 回顾性分析 11例人工晶体术后发生瞳孔夹持患者的临床资料。早期发生瞳孔夹持 5例 ,行非手术治疗。迟发的 6例中 ,2例患者的视力受影响较小 ,且无其他并发症 ,未予处理 ;4例行手术复位。结果 5例经非手术方法治疗后均复位成功。 4例手术复位的患者中 ,3例术后复位 ,并发症消失 ;1例因葡萄膜炎导致复位失败。结论 选择合适的人工晶体、撕囊口位置居中、人工晶体植入囊袋内和术毕缩小瞳孔等方法可有效地预防或减少瞳孔夹持的发生。术后早期发现瞳孔夹持 ,其非手术复位的成功率高 。

Objective To appraise the causes and preventive measures of intraocular lens pupillary capture(PC). Methods The clinical data of 11 eyes in 11 cases with pupillary capture after artificial lens implantation were analyzed retrospectively.5 eyes with early PC(occurring within 7 days after surgery) were managed by non-surgical method. 4 of 6 eyes with late PC were treated by surgical reposition.Results 5 non-surgical and 3 surgical eyes were repositioned successfully.Conclusion To select appropriate artificial lens, centrally placed capsulectomy, implant the IOL into the capsular bag and constrict the pupil after surgery will reduce the incidence of PC. Early detection and early reposition can prevent complications and achieve good vision.
