慢性肺原性心脏病25例心率变异性分析

Analysis of heart rate variability in 25 patients with chronic cor pulmonale

目的 通过心率变异性(HRV)时域分析和非线性分析评估慢性肺原性心脏病患者心脏自主神经功能损害。方法 采用24h动态心电图对25例慢性肺心病急性加重期患者和52例健康人监测,以HRV时域指标和Poincare散点图进行心率变异性分析。结果 肺心病组HRV各项时域指标(SDNN、SDSD、RMSSD、PNN50、WSD、WHI、SSD及SHI)和Poincare散点图定量指标(VLI、VAI)均明显低于正常对照组(P<0.01)。结论 慢性肺心病患者心脏自主神经功能严重受损,HRV分析可作为一种无创性评价慢性肺心病患者心脏自主神经功能的可靠方法。

Objective To evaluate cardiovascular autonomic function in patients with chronic cor pul-monale by 24 - hour heart rate variability (HRV) monitoring with time domain analysis and non - linear analysis. Methods 24 - hour electrocardiogram recordings were performed in 25 chronic cor pulmonale patients and 52 normal subjects for HRV analysis. Results SDNN, SDSD, RMSSD, PNN50, WSD, WHK SSD and SHI in chronic cor pulmonale patients were 67. 70 ±22.54 ms, 11.57 ± 4.83 ms , 20.29 ± 5.44ms and 1.96±1.03%, respectively. Both the time domain indices ( SDNN, SDSD , RMSSD and PNN50) and the Poincare plot indices (VLKVAI) were markedly reduced in chronic cor pulmonale patients compared to control subjects(p <0.01). Conclusions Results showed that patients with chronic cor pulmonale experience autonomic dysfunctions that may be responsible for heart failure and ventricular arrhythmias .
