一种改良中路法在颈内静脉穿刺术中的应用

Clinical application of modified middle-approach internal jugular vein catheterization

目的:为常规颈内静脉穿刺困难的患者提供寻求一种更好的颈内静脉穿刺方法。方法:将预计常规颈内静脉穿刺困难的100名患者随机均分为两组,试验组采用改良中路法,常规颈内静脉穿刺组作为对照组。比较两组穿刺成功率、穿刺时间、并发症发生率。结果:试验组49例(98%)穿刺成功,对照组39例(78%)穿刺成功(P<0.01);试验组和对照组穿刺时间分别为(4.3±3.0)min、(8.5±5.1)min(P<0.01);试验组1例(2%)误穿动脉,对照组7例(14%)误穿动脉(P>0.05)。结论:与传统穿刺法相比,改良中路法操作更简单快捷,穿刺成功率更高。

Objective To explore a better approach of internal jugular vein puncture for patients who is difficult to operate.Methods 100 Patients who were probably difficult to pucture were divided into two groups.The internal jugular vein puncture was performed by the modified middle-approach(test group) and conventional methods(control group) respectively,observing the successful rate,puncture time and complications in two groups.Results 49 patients in test group were successfully catheterized,the successful rate was 98%,and the 39 patients in control group were successfully catheterized,the successful rate was 78%(P <0.01).The puncture time was(4.3±3.0)min in test group,while(8.5±5.1) min in control group(P<0.01).The incidence of accidental puncture occurred in 2 cases(4%) in test group and 7 cases(14%)in control group(P>0.05).Conclusion Compared with conventional methods,modified middle approach puncture offers both convenience and high efficiency.
