蝶形椎板间开窗术治疗腰椎管狭窄症

Treatment of Lumbar Spinal Stenosis with Butterflylike Fenestration Decompression

目的 :观察蝶形椎板间开窗术治疗腰椎管狭窄症的手术疗效。方法 :采用蝶形椎板间开窗术治疗 4 0例腰椎管狭窄症。减压范围包括椎板间黄韧带及其所附着的上一椎板下 1/2、棘突基底部下 1/2骨质和下一椎板的外上缘包括小关节突内侧 1/3。结果 :随访 18个月～ 5年 ,平均 2年 4个月。根据评分标准 ,优 2 8例 ,良 9例。结论 :蝶形椎板间开窗术在保持腰椎稳定性的基础上最大限度地切除了腰椎管壁致狭窄病变组织 。

Objective:To observe the therapeutic effect of the surgical treatment of lumbar spinal stenosis with butterfly like fenestration decompression.Methods:40 cases of lumbar spinal stenosis were treated with butterfly like fenestration decompression.Result:The patients were followed up for 18～60 months,with average 28 months.The results were excellent in 28 cases,good in 9 cases.Conclusion:The author thought that the procedure removed the most of the interlaminal ligamentum flavum and minimal necessary bone,but offered sufficient decompression,less harm to lumbar stability.
