单侧外固定架治疗股骨干骨折

Treatment of femoral shaft fracture with unilateral external fixator

目的 :探讨外固定架治疗股骨干骨折的适应证和疗效。方法 :回顾性分析了 1996年 1月～ 2 0 0 1年 1月用外固定架治疗的股骨干骨折 5 6例 ,其中新鲜骨折 4 9例 ,陈旧骨折 7例。骨折粉碎程度按 Winquist分度 :0°11例 , °12例 , °15例 , °10例 , °8例。结果 :全部骨折都在 6个月内愈合 ,取掉外固定架。取架后 3个月复查 ,用改良 Thoeresen标准评定结果 :优 39例、良 14例、可 3例 ,优良率 94 .5 %。结论 :外固定架治疗股骨干骨折的最佳适应证是 °、 °开放性骨折 ,伴有感染的骨折 ,感染骨不连接 ,股骨干骨折合并血管损伤 ,儿童股骨干骨折 。

Objective:To investigate indications and curative effect in the treatment of femoral shaft fracture with unilateral external fixator.Methods:56 patients with femoral shaft fracture treated by unilateral external fixator between 1996 and 1999 were studies.According to the Winquist classification of femoral shaft fracture comminution,there were 11 cases of type 0 fracture,12 cases type Ⅰ fracture,15 cases type Ⅱ fracture,10 cases type Ⅲ fracture and 8 cases type Ⅳ fracture.Results:All fracture healed within 6 months.Accrding to modify criteria of Thoresen,the result of the treatment was evaluated as excellent in 39 patients,good in 14 patients and fair in 3 patients,the total excellent and good rate is 94.5%.Conclusion:External fixation indicated in some type 2 and most type 3 open fracture?infected fracture?infected nonunions,femoral shaft fracture assosiated with a vascular injury?femoral shaft and neek frature and it has also recently gained acceptance as treatment for closed femoral shaft frature in children.
