直肠印戒细胞癌和粘液腺癌临床病理分析

CLINIC AND PATHOLOGIC ANALYSIS OFTHE SIGNET-RING CELL CARCINOMA AND MUCINOUS ADENOCARCINOMA OF RECTUM

目的 :比较研究直肠印戒细胞癌和粘液腺癌临床和病理学特点的差异。方法 :回顾性地对比分析 2 0例直肠印戒细胞癌和同期 79例直肠粘液腺癌的临床和病理学特点。结果 :直肠印戒细胞癌组的一年生存率 6 5 .0 % ,三年生存率 30 .0 % ,五年生存率 5 .0 %。黏液腺癌组一年生存率 84 .8% ,三年生存率 5 8.2 % ,五年生存率 38.0 %。两者在显微镜下黏液分布不同 ,肉眼所见大体标本相差甚远。结论 :直肠印戒细胞癌较直肠粘液腺癌的诊断困难 ,恶性度高 ,预后差 ,临床上应对二者区别对待 ,术中冰冻切片有助于提高病理诊断准确率 。

Objective: TO study the difference in clini ca l and pathological features between the signet-ring cell carcinoma and mucinous adenocarcinoma of rectum. Methods: The clinical and pathological features of 20 signet-ring cell carcinom a and 79 mucinous adenocarcinoma of rectum were analysed. Results: The survival r ate one ,three and five years was 65.0% ,30.0% and 5.0% respectively in patients wi th signet-ring cell carcinoma of rectum; and 84.8% , 58.2% and 38.0% respectivel y in patients with mucinous adenocarcinoma of rectum. The distribution of mucus was different under microscope between signet-ring cell carcinoma and mucinous a denocarcinoma. Conclusion: The diagnosis is difficult, the survival ra te is very low and the malignant-degree is high in the signet-ring cell carcinoma of rectum . They should be treated discriminatingly in clinic, the correct pathological di agnosing-rate can be increased by the frozen-slice in the operation, so it is o f benefit to direct the operation.
