正颌术后硬组织位置变化预测的准确性评价

The Evaluation on the Accuracy of the Hard Tissue Changes for the Orthognathic Surgery.

目的 探讨牙颌面畸形患者行正颌外科手术术后硬组织位置变化与术前预测误差的影响因素。方法 对 15例牙颌面畸形患者正颌外科手术后 1个星期拍头颅定位侧位片进行头影测量 ,并和术前正畸后头颅定位侧位片模拟手术后所得预测值进行 15项硬组织测量指标统计分析。结果 术后测量值和术前预测值在水平向量变化上存在着绝对值差异 ,但无显著性差异 ,而在一些垂直向量变化中存在着显著性差异 ,分析差异引起的原因 :术前头影测量和模型外科的差异 ,牙合板制作不准确的影响 ,颞颌关节的影响及术中因素的影响。结论 术前精确的头影测量和模型外科 ,牙合板的精确制作 。

Objective To study the factors affected the accuracy of the hard tissue changes for the orthognathic surgery. Methods Lateral cepholographs of 15 subjects were taken before one week and one week after the operation. 15 items affiliated to hard tissue from the lateral cepholometric measurement were recorded and analysed. Results There was no statistical significant difference between the two teams of items in movements horizontally, but there was significant difference between the two teams of items in movements vertically. This study analysed the reasons of causing the difference. The reasons include the difference between the items of the cepholometric measument and the items of the model surgery, the effect of the inaccurate bite plate, the effect of the temporomandibular joint and the effect in operation. Conclusion The key of the ideal treatment results comprises precise cepholometric measurement and model surgery prior to surgery, use of accurate bite plate, choosing appropriate external point and performing orthognathic surgery carefully.
