胰腺囊实性肿瘤8例临床病理分析

Cystic and solid tumors of pancreas: a clinicopatholgic study of 8 case s

目的 了解胰腺囊实性肿瘤的免疫组化特点 ,分析其临床病理特点与预后的关系。方法 组织学 (HE、PAS法 )和免疫组化 (SP法 )。结果 8例患者 7例为女性 ,年龄 1 9～ 44岁 ,平均 2 9 1岁 ,手术后均无复发。肿瘤较大 ,平均直径 1 0cm ,有包膜 ,由实性与囊性坏死区混合组成。组织学上 ,瘤细胞大小形态较一致 ,核圆形或卵圆形 ,核异型不明显 ,核分裂象罕见 ,以肿瘤细胞围绕血管排列成假乳头突起为其特征 ,瘤细胞退变、出血、泡沫细胞和胆固醇裂隙常见。免疫组化 :8例α1 AT(+) ,5例vimentin(+) ,3例NSE(+) ,2例CK(+) ,2例CgA(+) ;CEA、actin、S 1 0 0、Syn均为 (- )。结论 胰腺囊实性肿瘤多发生于年轻女性 。

Purpose To observe the immunohistochemical f ea tures of cystic and solid tumors of the pancreas (CSTP), and to analyze the rela tionship between clinicopathologic features and prognosis Methods Eight cases of CSTP were studied using histologic (HE and PAS stains), and immunohistochemical methods. Results Seven patients were young adult females, 19～44 years in age (mean 29.1 years) without recurrence after tumor resection The tumors w ere larger in size, mean diameter 10cm, all encapsulated, and solid alternately with cystic Histologically, a uniform population of cells was seen The nucle i were oval, round without alypia, and rate of mitotic activity was evident Ps udopapillary structure with a fibrovascular core was remarkable Hemorrhage, fo am cells and cholesterol crystals were often found Immunohistochemically, the tumors were positive for α 1-AT (8 cases), vimentin (5 cases), NSE (3 cases ), CK and CgA (2 cases) but negative for CEA, actin, S-100 and synaptophysin in all the cases Conclusion CSTP is a clinicopathologic entity in young femal e patients with a benign clinical course
