摘要
采用放射过敏原吸附试验 (RAST )和酶联免疫吸附试验 (ELISA)测定 5 2例青霉素过敏病人血清特异性IgE、IgG抗体 ,进一步探讨青霉素过敏反应机制。结果 5 2例过敏病人特异性IgE、IgG抗体的阳性率分别为 5 0 %和 44 2 % ,若RAST与ELISA联合检测 ,IgE和IgG抗体总阳性率增至 63 5 %。荨麻疹组BPO IgG水平高于过敏性休克组 (P <0 0 1) ,过敏性休克病人BPA IgG水平明显高于BPO IgG (P <0 0 1) ,荨麻疹组内BPO IgE水平与BPA IgE无显著差异 (P >0 0 5 ) ,但均比过敏性休克组高。研究结果提示 ,荨麻疹与BPO IgE和BPA IgE关系密切 ,过敏性休克与BPO IgE和BPA IgG关系密切 ;同时检测IgE和IgG抗体 ,可提高诊断阳性率。
The radioallergosorbent test (RAST) and the enzyme linked immunosorbent assay (ELISA) were used to detect specific IgE and IgG antibodies in sera of 52 penicillins allergic patients so as to explore the mechanism of penicillin allergic reaction. Results Positive rates of specific IgE and IgG antibodies were 50% (26/52) and 44.2% (23/52), respectively. Positive rate of specific antibody was 63 5% with RAST combined with ELISA together. The level of IgG antibody specific to BPO PLL in the urticaria group was significantly higher than that of the anaphylactic shock group ( P <0.01). The level of IgG antibody specific to BPA PLL was significantly higher than the level of IgG antibody specific to BPO PLL in the anaphylactic shock group ( P <0.01). There was no distinctly different when the level of IgE antibody specific to BPO PLL compared with the level of IgE antibody specific to BPA PLL in the urticaria group ( P >0.05), but the levels of IgE antibodies specific to BPO PLL and BPA PLL in the urticaria group were higher than those of the anaphylactic shock group. Result suggests that the IgE antibodies specific to BPO PLL and BPA PLL are more associated with urticaia , whereas the IgE antibody specific to BPO PLL and the IgG antibody specific to BPA PLL are more associated with anaphylactic shock. Positive rate could be higher by detecting specific IgE and IgG together in the diagnosis of allergy to penicillins.
