国产异丙酚的临床应用及对维库溴铵肌松效应的影响

Clinical Use of Homemade Propofol and It's Effect on Neuromuscular Blockade of Vecuronium

目的 比较国产与进口异丙酚的麻醉效能、安全性及对维库溴铵肌松效应的影响。方法 将 60例择期行腹腔镜下胆囊切除手术的病人随机分成国产异丙酚组 (P组 )和进口异丙酚组 (D组 )。诱导用药为静注 1%利多卡因 1mg/kg后 ,分别静注国产和进口异丙酚 2mg/kg ,维库溴铵 0 1mg/kg。气管插管后静注芬太尼 4μg/kg ,分别以国产和进口异丙酚 4～ 8mg·kg-1·h-1输注维持麻醉。记录患者诱导时入睡时间、苏醒期间唤醒时间、完全清醒时间 ,循环变化 ,对维库溴胺肌松效应的影响及恶心、呕吐、燥动、注射处疼痛等不良反应。结果 两组入睡时间、唤醒时间、清醒时间相似。其他安全性指标如对血压、心率的影响和其他不良反应的发生率 ,两组无显著差异。对维库溴铵起效时间及维持时间的影响两组无显著差异。结论 临床应用国产和进口异丙酚在麻醉效能、安全性及对维库溴铵肌松效应影响方面无明显差异。

Objective To compare anesthetic efficacy,safety and its effect on neuromuscular blockade of vecuronium of homemade and import propofol.Methods Sixty adult patients,ASA classⅠ-Ⅱ,Scheduled for selective laparoscopic cholecystectomy,were asigned randomly into two group;homemade propofol group (group P),from England import propofol group(group D).By intravenous 1% lidocaine 1mg/kg and propofol 2mg/kg and vecuronium 0 1mg/kg,as anesthesia induction.After endotracheal intubation,fentanyl was given 4μg/kg intravenously, and propofol was infused at 4～8mg·kg -1 ·h -1 .Use American 10300 life signs monitor and TOF-GUARD accelerometer,we observed the going to sleep time during induction,the open eyes' time and completely lucid time during resuscitation.observed the blood pressure and heart rate change,neuromuscular blockade and side effects such as nausea,vomiting,uncontrolled moving,injection pain.Results There was no significantly differences between two groups in going to sleep time, open eyes'time and completely lucid time.There was no signifieantly differences in circulation changes,side effects occurance,neuromuscular blockade of vecuronium.Conclusions In clinical use, there is no significant differences between homemade propofol and import propofol in anesthetic efficacy,safety and neuromuscular blockade of vecuronium.
