电刺激膈肌起搏器恢复高位颈髓损伤患者的呼吸功能

摘要 高位颈髓损伤呼吸肌瘫痪而依赖呼吸机存活的患者不断增多,约占脊髓损伤的5%。气管切开长期机械通气并发症多,护理花费大。代替机械通气的唯一方法是应用电刺激膈肌起搏器。该装置包括在膈神经上安放电极,通过导线将电极与植入腹部皮下的电磁接收-刺激器接通,再经体外发射器为体内装置提供电能和参数指令。膈神经刺激器在国外已有1500多例的临床经验,效果良好。膈神经植入术神经假体恢复呼吸功能的研究与应用,包括呼吸动力肌、临床病倒选择、手术方法、术后调试训练、治疗效果,以及肋间神经电刺激、肋间神经移位于膈神经后再行电刺激等研究进展。
