乳头溢液的诊断与治疗(附360例分析)

Diagnosis and treatment of nipple discharge (360 cases analysis)

目的 探讨乳头溢液的诊断和治疗方法。方法 对360例乳头溢液患者进行回顾性分析。360例均为女性,年龄16～74岁,平均43岁。溢液时间3天～2.5年。单侧溢液者302例,双侧溢液者58例,伴乳房肿块者29例。选择其中82例进行手术治疗。结果 术后病理结果为乳腺癌18例,乳管内乳头状瘤36例,良性乳房疾病28例。65例乳头血性溢液患者中病理结果为乳癌者15例,占23.1％;乳管内乳头状瘤病32例,占49.2％。结论 乳头溢液涂片细胞学结合乳管造影、导管镜可明确乳头溢液原因。对45岁以上的乳头血性溢液患者应及时行手术治疗。

Objective To investigate the diagnosis and treatment of nipple discharge. Method 360 cases with nipple discharge are retrospectively studied. Results In the 82 cases with nipple discharge selected to operate, the pathological results are composed of carcinoma of breast 18, in-traductal popilloma of breast 36 and benign breast diseases 28.There are 15(23.1% ) carcinoma of breast and 32(49.2% ) intraductal popilloma of breast in the 65 cases with nipple bloody discharge. Condll-sions The reason of nipple discharge can be confirmed by exfoliate cytology combined with galactogram and fiberoptic ductoscopy.The operation should be performed in time in patients with nipple blood discharge over 45 years.
