不同眼轴长度白内障超声乳化吸出术临床观察 被引量:6

摘要 目的 评价不同眼轴长度眼的晶状体乳化吸出联合人工晶体植入术的效果。方法 不同眼轴长度189例200眼白内障采用超声乳化吸出人工晶状体植入。结果 (1)术后1周矫正视力≥0.5者在眼轴26.01~35.00mm组中占50%,在眼轴20.00~22.00mm组中占57.10%,在眼轴22.01~26.00mm组中占89.9%。(2)眼轴长26.01~30.00mm组中与眼轴长>30mm组中矫正视力≥0.5分别是77.8%、17.4%,两组比较差异有非常显著性(X^2=18.12,P<0.01)。(3)眼轴<21mm组中矫正视力≥0.5者无,21.01~22.00mm组中≥0.5者占66.7%,两组比较差异有显著性。(4)术中后囊破裂2眼(1%),角膜水肿14眼(7%)。结论 不同眼轴长度的眼超声乳化术后视力恢复程度不同,眼轴极长者(>30mm)和极短者(<21mm)视力恢复较差。 Objective To investigate the clinical effect of phacoemulsification with intraocular lens implantation to different axial length eyes. Methods We selected 200 eyes in 189 cataract patients including 71 eyes of male and 129 of female.They all underwent phacoemulsification with intraocular lens implantation through sclera tunnel incision.Results (l) Visual acuity of one week after the operation was beyond 0.5 in 152 eyes,including 25 eyes of 26.01 - 35.00 mm axial length and 12 eyes of 20-22.00 mm and 115 eyes of 22.01 - 26.00 mm.The percentage were 50% ,57.10% ,89.9% respectively. (2)The postoperative visual acuity beyond 0.5 were in 77.8% and 17.4% respectively of the patients 26.01-30.00 mm axial length and beyond 30.00 mm.The difference was significient(x2 = 18.12,P<0.01).(3)No postoperative visual acuity was beyond 0.5 in patients of axial length shortter than 21 mm while 66.7% existed in axial length of 21.01 -22.00 mm.(4)During and after the operation, posterior lens capsule ruptured in 2 eyes and 14 eyes with edema cornea. Conclusion Phacoemulsification with implantation of lOLs to different axial length eyes has the opportunity of quick recovery in vision and stability of refraction. But to patients whose axial length were too long ( > 30.00 mm) or too short ( < 21.00 mm) ,the effect was bad.
