白内障囊外摘出术中玻璃体脱出的原因及处理

Causes and management of the vitreous prolapse in cataract extrac- tion.

目的 分析白内障人工晶状体植入术中玻璃体脱出的原因,提出预防及处理方法。方法 273例(292眼)白内障常规进行现代囊外摘出术+人工晶状体植入术,术中32眼(10.96%)玻璃体脱出者行前部局限性玻璃体切除术。结果 分析玻璃体脱出原因:眼睑轮匝肌麻痹不良8眼;眼压高4眼;术中刺囊致后囊破裂5眼;注吸皮质及后囊抛光致后囊破裂15眼。结论 术前进行充分麻醉,有效地降低眼压及术中正确的操作,可以预防玻璃体脱出。

Objective To find out the causes of the vitreous prolapse in cataract extraction and take preventive measures and proper manage- ment.Methods 273 patients (292 eye) were operated with modern extraction and implantation of IOL, vitreous prolapse were found in 32 eyes. Results Of the 32 eyes with vitreous prolapse, anesthtic causes were considered in 8 eyes,high-IOL in 4 eyes, operatin manuvers fails in 20 eyes. Conclusion To prevent vitreous prolapse, patients should he well anaesthetized and the intraocular pressure should he lowered, also the correct manipulation should be made sure of.
