Metastatic Tumor of Posterior Fossa

作者对27例后颅窝转移瘤的临床资料进行了分析.男、女之比为3:1,年龄在40岁以上者24例.发现原发灶者11例,其中肺癌7例,鼻咽癌3例,肝癌1例。27例均有明显颅内压增高,多数患者伴有小脑体征及颅神经损害表现。27例中经手术治疗者26例,25例近期效果好转,术后死于肺部感染者1例.作者结合文献复习对诊断及治疗进行了讨论,强调了早期手术的重要性。

Twenty-seven cases of me-tastatic tumor of posterior fossa were pre-sented in this paper. Of the patients, 20were males and 7 females; their agerangedfrom 20 to 68, and 89% of them were over40 years of age. The primary foci foundwere bronchopulmonary carcinomas (7cases), rhinopharyngeal carcinomas(8 cases).and cancer of the liver (1case), but in16 cases the primary foci were no found.All of these patients presented symptomsand signs of obviously increased intra-cranial pressure, and most of them had thesigns of cerebellar and cranial nerves' le-sions. Ventriculorgraphy and brain CTscanning were important procedures in thediagnosis, and MRI was most valuable inestablishing the diagnosis and localizationof posterior fossa metastatic tumors. Twenty-six patients underwent operation. Theshort-term follow-up study showed animprovement in 25 cases. There was onedeath. The surgical treatment of posteriorfossa metastatic tumor was emphasized.Radiotherapy and chemotherapy were dis-cussed.
