客观营养指数在慢性心力衰竭患者营养评价中的应用价值 被引量:8

摘要 目的 :评价客观营养指数在心衰患者营养评价中的应用价值。方法 :收集100例年龄≥18岁[平均年龄(61.8±15.0)岁]慢性心衰的住院患者,以微型营养评价(MNA)为"金标准",评价控制营养状态评分(CONUT)、营养预后指数(PNI)、老年营养风险指数(GNRI)的特异性、敏感性以及对相关预后的预测性。结果:3种客观营养指数评价得到的心衰患者营养不良发生率分别为30%、31%、49%。效度分析中,GNRI的评价效果较准确可靠,约登指数为0.738,特异性、敏感性分别为0.922、0.816。且3种客观营养指数和人体测量指标、实验室营养指标均具有较高的相关性(P<0.05)。预测性分析中,Logistic回归模型显示:PNI、GNRI对住院日、营养相关并发症(感染)具有独立预测性,且营养指数和脑钠肽、左室射血分数(EF)等预后相关指标具有相关性(P<0.05),说明和疾病严重程度相关。结论:心衰患者的营养不良发生率高,准确评估心衰患者的营养状况有助于更好的评估预后相关信息。
