摘要 目的:通过比较分析Luminal A型与Luminal B型乳腺癌动态增强磁共振影像学征象,寻找Luminal A型与Luminal B型乳腺癌特征性MRI征象。方法:选取术前行MR扫描且病理证实为乳腺浸润性导管癌的196例女性患者。依据乳腺影像报告和数据系统(breast imaging reporting and data system,BI-RADS)磁共振部分评估乳腺浸润性导管癌的MRI征象,通过免疫组化取得ER、PR和HER-2的结果;采用卡方检验分析Luminal A型与Luminal B型的MRI征象有无显著性差异。结果:在287例乳腺癌中有Luminal型乳腺癌196例,其中Luminal A型143例,Luminal B型53例。环形强化(2=4.385,=0.036)和流出型曲线(χ2=11.397,=0.001)的比例在Luminal B型型乳腺癌增高;其他MRI征象在Luminal型和Luminal B型乳腺癌的比例均无统计学差异。结论:部分动态增强磁共振的形态学和血流动力学征象在Luminal A型与Luminal B型乳腺癌之间存在显著性差异;对术前制定治疗方案有所帮助,并能够进行治疗效果和预后评估。 Objective: To analysed the correlation between MRI features and breast cancer molecular subtype,and to found out the magnetic resonance imaging(MRI) features of Luminal breast cancer.Method:Before surgery,dynamic contrast-enhanced MRI was performed in 287 patients with breast cancer. Breast MRI imaging was assessed by Breast Imaging Reporting and Data System(BI-RADS) lexicon. ER、PR、HER-2 were detected with immunohistochemistry. Chi-square test was used to study the association of MR features with Luminal breast cancer.Results: Among 287 breast cancer cases, 196 patients were Luminal breast cancer,Among them Luminal A breast cancer were 143 and Luminal B breast cancer were 53. Rim enhancement(χ2=4.385, =0.036) and washout curves(χ2=11.397, =0.001)were significantly increased in Luminal B breast cancer. There was not statistically different between Luminal breast cancer and other MRI features. Conclusion: Some MRI features of Luminal A breast cancer and Luminal B breast cancer are significantly different, which can which can help decided treatment plan before operation,and can be used for estimating the treatment effect and prognosis.
