钢板内固定 皮质骨剥离 自体骨移植治疗骨不连

Treatment of Nonunion with Plate Fixation,Osteoperiosteal Decortication,and Bone Graft

摘要 目的:总结应用钢板内固定、皮质骨剥离及自体骨移植的方法治疗骨不连的疗效。方法:对16例骨不连患者行加压钢板、皮质骨剥离及自体髂骨植骨治疗,并行影像学随访,平均随访时间为7个月。结果:全部病例术后周身情况稳定。影像学结果显示术后3个月可见有连续性骨痂形成;6个月骨折基本愈合。除1例出现膝关节僵硬外,无感染、内固定物断裂等并发症发生。膝关节的平均活动度为103°。结论:钢板内固定、皮质骨剥离及自体骨移植是一种简单、有效的治疗骨不连的较好方法。 Objective:The purpose of this study was to review the results of plate fixation combined with osteoperiosteal decortication and grafting with bone from the iliac crest as treatment for nonunion.Methods:Sixteen patients who had had nonunion treated with plate fixation,osteoperiosteal decortication and autogenous bone grafting.The average time from the initial treatment of the fracture was fifteen months.Plates were used for fixation of the nonunion,with osteoperiosteal decortication and autogenous bone graft used in all patients.The average time from the repair of the nonunion to the latest follow- up examination was seven months.Results:By the time of the latest follow — up examination,all sixteen nonunions had healed.There was only one complication:loss of motion of one knee.The average range of motion of the knee at the latest follow-up examination was 103 degrees.Conclusion:Plate fixation,osteoperiosteal decortication,and autogenous bone grafting is an effective technique for the treatment of nonunion.
