暴发性阴囊坏疽的诊治

Discussion on the Diagnosis and Treatment of Foudroyant Gangrene of Scrotum

目的:探讨暴发性阴囊坏疽的临床诊治。方法:4例暴发性阴囊坏疽患者,对确已坏死的阴囊皮肤予以切除,尽量保留未坏死的皮肤及皮下组织,在阴囊皮肤脓肿及水肿面切开多个小切口引流,每日双氧水、雷弗诺尔纱条换药,根据药敏实验联合应用抗菌素。结果:3例愈合,1例死亡,切口愈合时间平均19 d。结论:暴发性阴囊坏疽是一种较凶险的疾病,致残率、致死率均较高,全身支持治疗在治疗中是非常重要的,决定治疗效果的关键是早期诊断,彻底清创[1]。

Objective:To study the clinical diagnosis and treatment of fulminant gangrene of scrotum.Methods:In four cases of fulminant gangrene of scrotum,to excise the necrotic scrotum skin and retain the intact derma,then make several incisive drainages on the dermapostasis and edema of scrotum followed by daily changed dressings using oxyful and Leifonuoer gauze.Results:Three patients are healed and one death while the wound healing requires 19 days on average.Conclusions:Fulminant gangrene of scrotum is a dangerous disease with high disability rate and mortality,so systemic therapy seems very important for such kind of treatment with its effect decided by early diagnosis and thorough debridement.
