摘要
目的研究分析胎儿椎体畸形的产前超声诊断价值。方法收集东莞市茶山医院与东莞市石碣医院2012年1月至2017年12月经产后或引产X线确诊的21例胎儿椎体畸形为研究对象,将相关二维及三维检查数据进行整理分析。结果 21例胎儿椎体畸形中,半椎体畸形14例,蝴蝶椎畸形5例,半椎体并蝴蝶椎畸形2例,其中单发椎体畸形7例,多发椎体畸形14例,单纯椎体畸形10例,合并其他系统畸形11例。产后或引产后X线检查证实共有39个椎体畸形,其中半椎体27个,蝴蝶椎12个,产前二维超声检出35个(90.48%),二维与三维超声联合检出39个(100%),两者比较,差异有统计学意义(χ~2=5.902,P<0.05)。结论胎儿椎体畸形的产前三维超声检查是二维超声检查进行有效的补充,二维联合三维超声检查能提高对椎体畸形及其伴发畸形诊断的准确性,应在临床中推广应用以减低严重椎体畸形人口的出生率。
Objective To study the value of prenatal ultrasound diagnosis of fetal vertebral deformity.Methods 21 cases of fetal vertebral malformation diagnosed by postpartum or induced X-ray were collected from Chashan Hospital of Dongguan City and Shijie Hospital of Dongguan City from January 2012 to December 2017.The relevant two-dimensional and three-dimensional examination data were analyzed.Results In the 21 cases of fetal vertebral malformation,14 cases were hemivertebra deformity,5 cases were butterfly vertebral deformity,2 cases were hemivertebra and butterfly vertebral deformity,including 7 cases of single vertebral deformity and 14 cases of multiple vertebral deformity.10 cases of vertebral deformity,11 cases with other system malformations.X-ray examination after birth or induction of labor confirmed a total of 39 vertebral malformations,including 27 vertebral bodies,12 butterfly vertebral bodies,35 prenatal two-dimensional ultrasound detection(90.48%),combined detection of two-dimensional and three-dimensional ultrasound 39(100%),the difference was statistically significant(χ~2=5.902,P<0.05).Conclusion Prenatal three-dimensional ultrasonography of fetal vertebral deformity is an effective supplement to two-dimensional ultrasonography.Two-dimensional combined three-dimensional ultrasonography can improve the accuracy of diagnosis of vertebral deformity and its associated malformation.It should be widely used in clinical practice.To reduce the birth rate of people with severe vertebral deformities.
