灰阶血流显像技术在动脉硬化病变中的诊断价值

摘要 目的探讨灰阶血流成像(B-Flow)技术在诊断动脉病变中的准确性和使用价值。方法对98例患者,共156条血管分别采用二维超声(2D)、彩色多普勒血流显像(CDFI)以及B-Flow进行常规动脉检查,观察血管壁、管腔及血流动力学改变;对动脉硬化及斑块形成患者再分别采用CDFI、B-Flow及二者联合模式下检查,留取相关图像,并对三种模式留取的图像进行对比分析。结果 B-Flow技术在对斑块部位、大小及对血管内径、血管壁厚度测量的准确性方面优于2D及CDFI;对98例患者129根动脉硬化患者中,采用CDFI、B-Flow及二者联合模式下检出动脉硬化病变分别为113根、125根及129根,3组间差异无统计学意义(P>0.05);2组超声对比检查对早期动脉硬化内膜增厚的检出差异有统计学意义(P<0.05)。结论 B-Flow扩展了二维影像显示血流的能力,提高图像质量及病变检出率,具有独特的临床应用价值;B-Flow与CDFI联合模式下,对动脉硬化患者,尤其早期病变并斑块形成患者检出率高,为临床提供更多、更准确的诊断信息。 Objective To evaluate the accuracy and value of B-Flow technique in the diagnosis of atherosclerotic lesions by comparing two-dimensional ultrasound( 2D),color Doppler flow imaging( CDFI) and bright-mode blood flow imaging( B-Flow). Methods The blood flow,wall and lumen of the artery were observed by conventional arterial examination with 2D,CDFI and B-Flow techniques. Comparative analysis of the images was also obtained. Results B-Flow technology for the display of soft plaque,blood vessel diameter and thickness measurement of vessel wall was more accurate than 2D and CDFI; A total of 129 atherosclerotic vascular from 98 patients were detected by CDFI,B-Flow,and the combined group were 113,125 and 129,respectively( P > 0. 05); There were significantly difference between three groups on detecting early atherosclerosis intimal thickening( P < 0. 05). Conclusion B-Flow combined with 2D and CDFI can significantly improve image quality,and provide more accurate diagnostic information in the diagnosis of vascular disease; B-Flow and CDFI combined mode can provide more information for clinical diagnosis with more accurate on arteriosclerosis patients.
