摘要
目的探讨伤安素(OUR)碳纤维敷料与磺胺嘧啶锌软膏联合治疗人体受压部位深度烧伤的效果。方法选择深度烧伤创面波及背、臀与四肢背侧面受压部位患者119例,均为深Ⅱ度以上创面。按入院单双日将患者随机分为对照组(56例)涂磺胺嘧啶银覆盖棉垫,治疗组(63例)涂磺胺嘧啶锌软膏覆盖OUR碳纤维敷料。2组患者均24 h换药1次,3 d后创面分泌物常规做细菌培养+药敏试验。观察48 h内患者舒适度,外层覆盖物渗出情况,创面炎症反应情况,创面愈合时间,创面愈合质量。结果治疗组舒适度(安静配合治疗)占92.06%,外层覆盖物渗出情况(24 h浸透)占7.91%,创面炎症反应情况(细菌阳性)占33.33%,与对照组比较差异有统计学意义(P<0.05),创面愈合时间较对照组短(P<0.05)。结论 OUR碳纤维敷料具有吸附能力强,可抑制细菌生长,与磺胺嘧啶锌软膏联合应用具有抗感染性强,能促进创面上皮细胞生长,加速创面愈合、廉价,值得普及应用等优点。
Objective To observe the combined therapeurical effects of carbon fiber wound dressing named Shangansu and sulfadiazine zinc ointment on deep burn wounds of pressure area. Methods 121 patients with deep burn wounds of back,hip and limbs were selected and randomly divided into control group( wound application dressing with sulfadiazine silver and cotton mat) and treatment group( wound application dressing with sulfadiazine zinc ointment and Shangansu). The wound depth were above Ⅱ° and the areas were10% ~ 56%( 16. 84 ± 5. 34) TBSA. The wound dressing was changed once a day. The conventional bacterial culture and drug sensitive test for wound secretion were performed. The patient comfort degree,outer covering exudation,wound inflammation,wound healing time and wound healing quality were observed. Results 92. 06% patients on comfort degree( unrelieved treatment),7. 91% patients on outer covering exudation( 24 hours permeation rates),and 33. 33% patients on wound inflammation( positive rate of bacteria) in the treatment group,the percentage were significantly higher than the control group( χ2= 8. 206,8. 246,6. 706,P < 0. 05); Compared with the control group,the treatment group patients showed a higher comfort degree,no saturated covering of Shangansu after replacement for 24 h,a light residue wound inflammation,and a reduced wound healing time by 4. 76 days. Conclusion The carbon fiber wound dressing of Shangansu has an advantage of excellent exudate absorption and inhibition of bacterial growth. The combination application of Shangansu and sulfadiazine zinc ointment show an improved anti-infective performance,and can promote the growth of epithelial cells and wound healing.
