代谢综合征患者左房重构及其与N末端脑钠肽前体关系的探讨

Left atrial remodeling in patients with metabolic syndrome and its relationship with the N terminal brain natriuretic peptide precursor

目的了解代谢综合征患者左房重构及其与N末端脑钠肽前体(NT-pro BNP)水平间的关系。方法选择健康对照者30例,同时选择明确诊断为代谢综合征的患者30例,另外选择同时符合代谢综合征并高血压病诊断的患者30例。采用超声心动图测量3组患者左心房容积指数(LAVI)、左心房最大容积(LAVmax)、左心房最小容积(LAVmin)、左房储存器容积(LARV)、左房管道容积(LACV)、左心房主动收缩排空容积(LASV)、左房射血分数(LAEF)、左心房开始收缩容积(LAVP)、左心室最大容积(LVVmax),并应用ELISA试剂盒测定3组患者的血清NT-pro BNP水平,然后对上述检测结果行统计学分析。结果与健康对照组相比,代谢综合征组患者LAVI、LAVmax、LAVmin、LARV、LASV、LAVP增加,LACV、LAEF、LVVmax差异无统计学意义;与健康对照组和代谢综合征组相比,代谢综合征并高血压病组LAVI、LAVmax、LAVmin、LARV、LACV、LASV、LAVP、LVVmax均增加,LAEF差异无统计学意义。与健康对照组相比,代谢综合征组患者血清NT-pro BNP增加;与健康对照组和代谢综合征组相比,代谢综合征并高血压病组NT-pro BNP均增加。代谢综合征患者左房重构指数与血清NT-pro BNP存在正相关性。结论代谢综合征患者存在左房重构、血清NT-pro BNP增加;代谢综合征患者左房重构与其血清NT-pro BNP水平呈正相关性。

Objective To explore Left atrial remodeling in patients with metabolic syndrome and its relationship with the N terminal brain natriuretic peptide precursor. Methods To choose 30 cases of healthy controls and 30 patients with metabolic syndrome clearly diagnosed,and then choose 30 cases of patients with metabolic syndrome with high blood pressure. Three groups were measured with echocardiography in order to observer left atrial volume index( LAVI),left atrial maximum volume( LAVmax),left atrial minimum volume( LAVmin),left atrial reservoir volume( LARV),left atrial volume pipe( LACV),the active contraction of the left atrial emptying volume( LASV),left atrial ejection fraction( LAEF),left atrial contraction volume( LAVP) and left ventricular volume( LVVmax),The serum levels of NT- pro BNP line were detected by Elisa kit and the result were statistical analyzed. Results To compare with the healthy control,LAVI,LAVMAX,LAVMIX,LARV,LASV increased in patients with the metabolic syndrome group,and LAVP,LACV,LAEF,LVVmax were no difference. To Compare with healthy control group and metabolic syndrome group,LAVI,LAVMAX,LAVMIX,LARV,LACV,LASV,LAVP,LVVmax increased in the metabolic syndrome and hypertension disease groupand LAEF was no difference. To compare with healthy control group,serum NT- pro BNP increased in patients with the metabolic syndrome group; To compared with healthy control group and metabolic syndrome group,NT- pro BNP increased in metabolic syndrome and hypertension disease group. The index of left atrial remodeling in patients with metabolic syndrome is positively correlated with serum NT- pro BNP. Conclusion The metabolic syndrome in patients had left atrial remodeling; The serum NT- pro BNP increased in the metabolic syndrome patients; Left atrial remodeling in patients with metabolic syndrome and its NT- pro BNP serum levels were positively correlated.
