血清缺氧诱导因子-1α水平与IgA肾病进展危险因素关系的研究

Relationship between the level of hypoxia-inducible factor-1α and the risk factors for progression of IgA nephropathy

目的探讨缺氧诱导因子-1α(HIF-1α)与原发性IgA肾病(IgAN)进展危险因素的相关性。方法收集85例IgAN患者临床资料,检测血HIF-1α水平,以肾功能是否异常分为2组,比较2组患者间指标的差异,并将血HIF-1α与肾病进展的危险因素进行相关性分析。结果 2组患者年龄、24 h尿蛋白定量、肾小球滤过率(eGFR)、血肌酐、血尿酸、高血压比例及血HIF-1α水平间比较差异有统计学意义(P<0.05);HIF-1α与eGFR呈负相关性(r=-0.652,P<0.05),与24 h尿蛋白定量呈正相关性(r=-0.659,P<0.05);HIF-1α的ROC曲线分析提示曲线下面积为0.761(P<0.05),HIF-1α诊断肾功能异常的临界点为147 pg/L,尤登指数为0.4。结论血HIF-1α对肾功能损害有一定的预测作用,同时与IgAN进展的危险因素有相关性,临床上可以关注血HIF-1α水平以判断IgAN的预后。

Objective To investigate the relationship between hypoxia inducible factor-1α(HIF-1α)and risk factors for primary IgA nephropathy(IgAN).Methods A retrospective analysis of 85 patients with IgAN diagnosed were selected.The data were collected and the HIF-1αlevels in the blood were measured.The patients were divided into two groups according to the renal function.The differences of indicators in two groups were compared and the HIF-1αwas analyzed the correlation with the risk factors for the progression of nephropathy,and the ROC curve of HIF-1αwas plotted.Results There were statistical differences between the two groups in terms of age,24-hour urine protein quantitation,eGFR,serum creatinine,blood uric acid,the proportion of hypertension(P<0.05).HIF-1αwas negatively correlated with eGFR(r=-0.652,P<0.05),and There was a positive correlation with 24 h urine protein(r=-0.659,P<0.05).The ROC curve of HIF-1αindicated that the area under the curve was 0.761(P<0.05),and the critical point for diagnosing renal dysfunction of HIF-1αwas 147 pg/L.The Youden Index was 0.4.Conclusion Blood HIF-1αhas a certain predictive effect on renal dysfunction,and it is related to the risk factors of IgAN progression.Clinically,we can pay attention to the level of blood HIF-1αto judge the prognosis of IgAN.
