血清Presepsin、PCT、CRP联合检测在脓毒症诊断中的价值

The value of combined detection of serum Presepsin,PCT and CRP in the diagnosis of sepsis

目的通过联合检测血清中Presepsin、降钙素原(PCT)、C反应蛋白(CRP)、IL-6水平,探讨血清标记物联合检测对脓毒症的诊断价值。方法选取30例脓毒症、20例严重脓毒症、30例非感染SIRS患者及体检健康者30例,分别监测血清Presepsin、PCT、CRP、IL-6表达水平,分析表达差异性与APACHEⅡ的相关性,并采用ROC曲线分析单个及联合检测对脓毒症患者的诊断价值。结果非感染SIRS患者与脓毒症患者间血清Presepsin、PCT、CRP水平差异均有统计学意义(P<0.05);IL-6水平无差异性(P>0.05);Presepsin、PCT、CRP的含量与APACHEⅡ具有强正相关性(P<0.05);Presepsin、PCT、CRP、IL-6的AUC值分别为0.876、0.851、0.784、0.548;Presepsin、PCT、CRP三者联合诊断的AUC值为0.957,高于单独及其他联合分组。结论血清Presepsin、PCT和CRP三者联合检测能提高脓毒症患者的诊断率。

Objective To investigate the diagnostic value of combined detection of serum markers in sepsis by combined detection of serum levels of Presepsin,PCT,CRP and IL6.Methods Thirty patients with sepsis,20 patients with severe sepsis and 30 patients with non-infected SIRS were enrolled.Serum levels of Presepsin,PCT,CRP and IL6 were monitored,and the difference in expression was analyzed and correlated with APACHEⅡ.The ROC curve was used to analyze the value of single and combined detection in patients with sepsis.Results There were significant differences in serum Presepsin,PCT and CRP between non-infected SIRS patients and sepsis patients(P<0.05).There was no difference in IL6 levels(P>0.05).Presepsin,PCT and CRP levels were positive correlated with APACHEⅡ(P<0.05);the AUC values of Presepsin,PCT,CRP,and IL6 were 0.876,0.851,0.784,and 0.548,respectively;the combined diagnostic AUC values of Presepsin,PCT and CRP were 0.957,which were all higher than that of the combined group alone and other groups.Conclusion The combined detection of serum Presepsin/PCT/CRP can improve the diagnosis rate of patients with sepsis.
